Latar Belakang: Upaya meningkatkan pemera taan dan mutu pelayanan kesehatan melalui puskesmas, Dinas Kesehatan Kabupaten Sawahlunto/Sijunjung mengembangkan Puskesmas Muaro Bodi menjadi puskesmas unit swadana. Sejak penerapan swadana terjadi peningkatan jumlah kunjungan menjadi dua kali lipat dan peningkatan pendapatan yang melampaui target. Keberhasilan puskesmas menerapkan swadana terlihat dari performance dan perubahan manajemen keuangan puskesmas tersebut. Penelitian ini bertujuan memperoleh informasi peningkatan produktifitas, peningkatan pendapatan, kepuasan pelanggan dan pengembangan sumber daya manusia serta perubahan manajemen keuangan Puskesmas dalam penerapan unit swadana. Metode: Penelitian ini merupakan penelitian deskriptif dengan rancangan studi kasus. Sumber data adalah hasil wawancara mendalam terhadap Kepala Puskesmas dan dokumen Puskesmas Muaro Bodi tahun 2002- 2004. Analisa data menggunakan analisa rangkaian waktu (Analysis of Time Series ) dengan metode kwadrat terkecil (least square method). Hasil: Setelah menerapkan swadana, produktifitas pelayanan, pendapatan, kepuasan pelanggan, dan pengembangan sumber daya manusia Puskesmas Muaro Bodi mengalami peningkatan. Terjadi perubahan proses manajemen keuangan puskesmas yang lebih mandiri, akurat, dan transparans. Puskesmas menerima wewenang menyusun anggaran kebutuhannya sendiri, tidak lagi bergantung dengan Dinas Kesehatan. Sistem satu pintu yang diterapkan memudahkan puskesmas dalam mengelola pendapatan. Kesimpulan: Penerapan puskesmas unit swadana memberikan pengaruh besar terhadap performance dan manajemen keuangan sebuah puskesmas. Puskesmas unit swadana mampu memecahkan masalah yang berkaitan dengan pembiayaan operasional puskesmas.

Background: In effort to increase distribution evenly and health service quality through community health centers, The Regency Health Office of Sawahlunto/Sijunjung developed Muaro Bodi community health center becoming a Self-Funded ones. Since the implementation of Self-Funded system, there are increasing amount of people visited to get treatment and beneficially increasing revenue at this community health center. The success on implementing this Self-Funded system reflected in their performance and their improvement on financial management. This research objectives was to obtained informations of improvement in productivity, revenue, costumer satisfactory, human resources development and their financial management at the Muaro Bodi community health center during implementation of Self-Funded system. Methods: This research was descriptive research with case study design approach. Sources of data were collected from recorded in depth interviews with the head of Muara Bodi community health center and including their documents from 2002 -2005. Data analyzed based on time series analyses using least square methods. Results: After implementing Self -Funded system, the productivities, services, revenues, costumer satisfactories, and human resources developments at the Muaro Bodi community health center have been increased. Even their financial management have become more self sustain, more accurate and more transparent. The one gate system that implemented eased community health center in running and managing their own revenues. Conclusion: Implementation of Self-Funded system in community health centers gave huge significant effect on their performance on public. It also provide solutions for problems connecting with their operational cost burden.
