Drug-Related Problem pada penatalaksanaan pasien rawat inap di RSAL Dr. Ramelan Surabaya Periode 1 September- 31 Oktober 2006

Latar belakang : Stroke merupakan penyakit yang memerlukan perawatan jangka panjang sehingga untuk mendapatkan therapeutic outcome yang baik perlu kerjasama antara dokter, perawat, apoteker, pasien dan keluarga pasien. Kejadian drug related problems sangat umum terjadi pada pasien rawat inap yang beresiko meningkatkan kesakitan, kematian dan biaya. Tujuan : Penelitian mengenai Drug Related Problems Pada Penatalaksanaan Pasien Stroke Rawat Inap di RSAL Dr. Ramelan Surabaya yang dilakukan ini bertujuan untuk menelusuri kejadian DRPs pada pasien stroke rawat inap dan untuk mengetahui apakah terjadi DRPs pada penatalaksanaan pasien stroke rawat inap di RSAL Dr Ramelan Surabaya periode 1 September – 31 Oktober 2006. Metode : Penelitian dilakukan dengan rancangan studi cross-sectional yang dikerjakan secara prospektif terhadap suatu populasi terbatas, yaitu seluruh pasien stroke rawat inap di RSAL Dr. Ramelan Surabaya periode 1 September-31Oktober 2006. Data pasien stroke rawat inap didapat dari poli saraf dan UGD, kemudian dilakukan pencatatan status pasien dari rekam medik di bangsal rawat inap. Kekurangan rekam medik dilengkapi dengan melihat catatan perawat, melihat kondisi pasien langsung dengan mengikuti visite dokter, dan wawancara pasien atau keluarga pasien. Data yang diperoleh dilakukan kajian DRPs dengan acuan guidelines stroke yang ada, dihitung persentasenya. Hasil dan Kesimpulan : Dari 109 pasien stroke rawat inap sebanyak 102 pasien memenuhi kriteria inklusi. Pada 102 pasien didapat 67 pasien mengalami DRPs (65,69%) dan 35 pasien tidak mengalami DRPs (34,31%). Jenis DRPs yang terjadi adalah drug needed 27 pasien (22,50%), wrong drug/innappropriate drug 32 pasien (26,67%), wrong dose 26 pasien (21,67%

Background: Stroke is a disease which requires long term care so that good cooperation among doctors, nurses, pharmacists, patients and patients' family is needed to get good therapeutic outcome. Drug related problems (DRP) generally occur among inpatients. They may increase risks of illness, death and cost. Objective: The study was meant to find out occurrence of DRP among stroke inpatients and identify whether there were drug related problems in the treatment of stroke inpatients at Dr. Ramelan Navy Hospital Surabaya during 1st September - 31st October 2006. Method: The study was an analytical cross sectional design conducted prospectively to limited population, i.e. all stroke inpatients at Dr. Ramelan Navy Hospital Surabaya from 1st September - 31st October 2006. Data of stroke inpatients were obtained from Neurology Polyclinic and Emergency Unit; then the recording of the patient's status was made from medical records of inpatients word. Incomplete medical records were minimized by looking at nurses' notes, the condition of the patient by following doctors' visits, interview with the patient and patient's family. Data obtained were studied on DRP using stroke guideline and calculated their percentage. Result and Conclusion: Out of 109 stroke patients, as many as 102 fulfilled inclusion criteria. Out of 102 patients, 67 patients (65.69%) had drug related problems, 35 patients (34.31%) did not have drug related problems. Types of drug related problems were drug needed 27 patients (22.50%), wrong drug / inappropriate drug 32 patients (26.67%), wrong dose 26 patients (21.67%), adverse drug reaction 16 patients (13.33%), drug interaction 19 patients (15.83%).
