Pengaruh strata sosial ekonomi dan kesehatan terhadap permintaan pelayanan persalinan di Indonesia :: Kasus DIY, Sultra, dan NTB

Studi ini melihat pengaruh karakteristik sosial ekonomi dan karakteristik kehamilan seperti apa yang dimiliki individu atau keluarga untuk memilih jenis pelayanan persalinan. Jenis pelayanan yang dimaksud adalah pilihan lokasi persalinan, pilihan alat transportasi, jenis kelamin penolong persalinan, pilihan penolong persalinan, pilihan tindakan medis, dan pilihan fasilitas pelayanan. Secara khusus, studi ini bertujuan untuk mengetahui probabilitas pilihan alternatif pelayanan yang disukai oleh ibu hamil atau melahirkan berdasarkan tingkat pendapatan, pendidikan, profesi atau jenis pekerjaan, urutan kehamilan (kehamilan pertama atau lainnya), usia ibu, dan risiko kehamilan. Model analisis yang digunakan dalam studi ini adalah model logit (the logit model) yang bertumpu pada fungsi probabilitas kumulatif. Estimasi dengan model ini ditujukan untuk menemukan nilai terbaik bagi masing-masing koefisien. Secara signifikan terdapat beberapa variabel yang mempengaruhi jenis pelayanan persalinan. Pilihan lokasi persalinan yang berdekatan dengan rumah/tempat tinggal banyak ditentukan oleh peran ibu dengan status pekerjaan; wiraswasta, memiliki tingkat pendidikan SMA, Diploma dan karakteristik daerah. Alat transportasi yang digunakan kelokasi persalinan dengan kendaraan khusus, faktor penentunya adalah status pekerjaan yang dimiliki ayah dan ibu serta karakteristik daerah. Status pekerjaan yang dimaksud adalah ayah sebagai karyawan BUMN/BUMD, karyawan swasta, dan ibu berstatus karyawan swasta. Pilihan penolong persalinan adalah perempuan, sangat kuat dipertimbangkan pada saat kehamilan pertama, ibu berpendidikan SMP dan ayah sebagai petani serta karakteristik daerah. Sementara itu, pilihan penolong persalinan dengan tenaga medis diduga dipegaruhi oleh status pekerjan ibu sebagai karyawan swasta dan tingkat pendidikan ibu S1 dan SMA serta ayah yang berpendidikan S1 serta karakteristik daerah. Penentuan jenis tindakan medis dengan cara caesar, hanya dapat ditentukan oleh dokter berdasarkan kondisi kesehatan ibu hamil. Karakteristik daerah tidak signifikan dalam penentuan jenis tindakan medis. Tingginya probabilitas untuk memilih fasilitas pelayanan persalinan yang khusus (VIP/berkelas) terjadi pada keluarga dengan status pekerjaan ayah sebagai karyawan BUMN/BUMD dan pada karakteristik daerah.

This study examines the characteristics of social and economy of individual or family related to delivery facility of pregnancy. The kinds of delivery facility in this study are the choice of location, the person who will deliver the delivery facility, the transportation they will use, the gender of doctor or nurse who will help of delivery facility and the kinds of medical services and facilities they want. This study aims to address the alternative probability of services that are chosen by the pregnant mothers based on the level of income, education, profession, age, risk of pregnancy and the level of pregnancy (the first, second or others). The logit model will be used in this study, which is based on the function of accumulation probabilities. The estimation of this model is illustrated by each coefficient of variables. There are some variables that significantly influence delivery facility. Delivery facility location choice near by where they live is chosen by private businessman mother, mother with high school education, private businesswoman mother, mother with diploma degree, and characteristics of each region. Particular transportation to delivery facility location choice is BUMN/BUMD employee father, private employee father, private employee mother and the characteristics of region. Selection in woman as delivery helper is mother with first pregnancy, mother with junior high school education, father as a farmer, and mother with bachelor degree . Selection in delivery helper by medical staff is private employee mother, mother with bachelor degree, mother with high school education, father with bachelor degree, and region's characteristics. Choice using caesar in delivery facility is chosen doctor based on healthy pregnancy patient. The region's characteristics are not significant in influencing the chosen of medical services. Special service facility is chosen BUMN/BUMD employee father and region's characteristics
