Hasil: Berdasarkan nilai cut-off rasio sTfR/log feritin > 3,2, sebanyak 35 subjek (21,6%) tergolong mengalami defisiensi besi. Analisis korelasi Spearman menunjukkan bahwa MCV (r = -0,523; p < 0 xss=removed xss=removed xss=removed>

Background: Routine blood donation causes significant iron loss and increases the risk of Non-Anemic Iron Deficiency (NAID), which frequently goes undetected by standard hemoglobin screening. Serum ferritin is the primary marker for iron stores; however, it is influenced by acute-phase responses or inflammation. The soluble transferrin receptor (sTfR)/log ferritin ratio is a more sensitive, inflammation-stable indicator of iron status, but its widespread use is hindered by high costs and limited accessibility. Erythrocyte indices (MCV, MCH, MCHC, and RDW) are routine, inexpensive, and easily accessible laboratory tests that have the potential to serve as early screening tools if proven to correlate well with iron status.

Objective: To analyze the correlation between erythrocyte indices (MCV, MCH, MCHC, and RDW) and the sTfR/log ferritin ratio in routine blood donors. Methods: This cross-sectional study was conducted at PMI Yogyakarta City and the Department of Clinical Pathology, FK-KMK UGM using secondary data collected from September to December 2024. Study subjects were routine whole blood donors (minimum 2 donations in the past year) who met the inclusion criteria. Samples were selected using consecutive sampling, yielding 162 subjects. Measured variables included erythrocyte indices (MCV, MCH, MCHC, RDW-CV) and iron status parameters (ferritin, sTfR, and sTfR/log ferritin ratio). Correlation was analyzed using Spearmans rank correlation test with a significance level of p < 0>

Results: Based on a sTfR/log ferritin ratio cut-off > 3.2, 35 subjects (21.6%) were identified as iron deficient. Spearman correlation analysis demonstrated moderate negative correlations between the sTfR/log ferritin ratio and MCV (r = -0.523; p <0 xss=removed xss=removed xss=removed>
