Cost Effectiveness Analysis Parecoxib Dibandingkan Ketorolak Pada Pasien Pasca Bedah Ortopedi di RSUD Kota Mojokerto

Nyeri pasca bedah merupakan permasalahan yang sering terjadi setelah tindakan operasi ortopedi. Parecoxib dan ketorolak merupakan analgesik golongan NSAID (Nonsteroidal Anti-Inflammatory Drugs) yang memiliki efektivitas dan efek samping yang berbeda, sehingga dapat memengaruhi beban biaya perawatan kesehatan di rumah sakit. Penelitian ini bertujuan untuk menganalisis cost effectiveness parecoxib dibandingkan ketorolak pada pasien pasca bedah ortopedi.

Penelitian ini dilakukan secara observasional analitik menggunakan rancangan penelitian kohort prospektif pada 64 pasien yang menjalani operasi ortopedi di RSUD Kota Mojokerto, dengan masing-masing kelompok terdiri dari 32 pasien. Outcome klinis berupa efektivitas analgesik yang dinilai dari jumlah pasien yang mencapai skala nyeri ringan (NRS ?3) pada jam ke-12 yang diukur menggunakan Numeric Rating Scale (NRS) dan kejadian efek samping obat. Analisis biaya diambil dari biaya medik langsung yang dihitung selama 24 jam pengamatan. Selanjutnya dilakukan studi farmakoekonomi menggunakan metode Cost Effectiveness Analysis (CEA). Analisis efektivitas biaya dilakukan berdasarkan perspektif rumah sakit dengan menghitung biaya medis langsung. Efektivitas biaya dihitung berdasarkan nilai Average Cost-Effectiveness Ratio (ACER) dan Incremental Cost-Effectiveness Ratio (ICER).

Kelompok parecoxib menunjukkan proporsi pasien yang mencapai nyeri ringan (NRS ?3) pada jam ke-12 lebih tinggi dibandingkan kelompok ketorolak, yaitu 84,4% (27 pasien) vs 75,0% (24 pasien), yang menunjukkan efektivitas analgesik lebih tinggi kelompok parecoxib, namun perbedaan tersebut tidak bermakna secara statistik (p=0,351). Efek samping tidak ditemukan pada kelompok parecoxib, sedangkan pada kelompok ketorolak terjadi efek samping pada 3 pasien (9,4%) yaitu mual muntah pada 2 pasien dan reaksi alergi pada 1 pasien. Hasil cost-effectiveness menunjukkan parecoxib membutuhkan biaya lebih tinggi (Rp831.513 ± Rp127.917 vs Rp582.980 ± Rp91.847) dengan proporsi pasien yang mencapai nyeri ringan lebih besar dan nilai ICER Rp2.643.968 per tambahan 1?ektivitas.

Postoperative pain is a common problem in patients undergoing orthopedic surgery. Parecoxib and ketorolac are nonsteroidal anti-inflamatory drug (NSAID) analgesics that differ in terms of effectiveness and adverse effects, which may impact hospital case costs. This study aims to analyze the cost-effectiveness of parecoxib compared with ketorolac for analgesia in postoperative orthopedic patients.

This analytical observational study employed a prospective cohort design involving 64 patients undergoing orthopedic surgery at Mojokerto City Hospital, with 32 patients in each group. Clinical outcomes included analgesic effect, assessed by proportion of patients achieving mild pain with Numeric Rating Scale (NRS ?3) at 12 hours postoperatively, and the incidence of adverse drug events. Subsequently, a pharmacoeconomic study was conducted using the Cost-Effectiveness Analysis (CEA) method. The cost effectiveness analysis was conducted from the hospital's perspective by calculating direct medical costs. Cost-effectiveness was calculated based on the Average Cost-Effectiveness Ratio (ACER) and Incremental Cost-Effectiveness Ratio (ICER).

The parecoxib group showed a higher proportion of patients achieving mild pain (NRS ?3) at 12 hours compared with the ketorolac group, at 84.4% (27 patients) versus 75.0% (24 patients), indicating greater analgesic efficacy with parecoxib, although the difference was not statistically significant (p=0.351). No adverse events were observed in the parecoxib group, whereas 3 patients (9.4%) in the ketorolac group experienced adverse events, including nausea and vomiting in 2 patients and an allergic reaction in 1 patient. The cost-effectiveness analysis showed that parecoxib was more costly than ketorolac (Rp831,513 ± Rp127,917 vs. Rp582,980 ± Rp91,847), with a higher proportion of patients achieving mild pain and an ICER of Rp2,643,968 per additional 1?fectiveness.
