Perbedaan Functional Brain Connectivity pada Penderita Alkoholisme Berbasis EEG: Analisis Coherence/PLV dan Graph Theory

Gangguan penggunaan alkohol (alcohol use disorder, AUD) merupakan beban kesehatan global yang deteksinya sebagian besar masih mengandalkan kuesioner subjektif, sehingga penanda objektif berbasis otak dibutuhkan. Functional connectivity dari sinyal EEG berpotensi mengisi kebutuhan tersebut, tetapi temuan terdahulu belum konsisten da lam hal pita frekuensi, wilayah, dan arah perubahan, sebagian karena ketergantungan pada satu metrik dan pengaruh konduksi volume. Penelitian ini mengkarakterisasi perbedaan functional connectivity otak antara penderita alkoholisme dan kontrol sehat pada empat pita frekuensi.
Analisis menggunakan dataset EEG publik UCI sebanyak 121 subjek, yaitu 76 penderita dan 45 kontrol, pada 19 kanal region of interest untuk pita theta, alpha, beta, dan gamma. Konektivitas dihitung dengan coherence dan phase locking value (PLV) sebagai metrik utama serta weighted phase lag index (wPLI) sebagai pembanding yang tahan konduksi volume. Perbedaan antarkelompok diuji pada tingkat edge untuk ketiga metrik dan pada tingkat jaringan melalui teori graf atas jaringan PLV berbobot yang diambang pada densitas 0,30. Inferensi statistik menggunakan uji Mann–Whitney U, ukuran efek Cliff’s delta, koreksi false discovery rate (FDR), uji permutasi 5.000 iterasi, dan korelasi Spearman antarmetrik.
Perbedaan tingkat edge terpusat pada pita alpha di wilayah frontocentral, dengan 23 edge PLV dan 22 edge coherence yang bertahan setelah koreksi FDR. Sebagian besar berupa hypoconnectivity pada kelompok penderita, dengan FC5, Cz, dan C3 sebagai kanal yang paling sering terlibat. PLV dan coherence sangat konsisten (Spearman 0,978–0,984), sedangkan wPLI tidak menghasilkan edge signifikan sehingga berperan sebagai batas konservatif. Pada tingkat jaringan, lima metrik graf pita theta berbeda signifikan, yaitu clustering coefficient dan global efficiency lebih tinggi serta path length lebih pendek pada kelompok penderita (Cliff’s delta 0,296–0,479). Pita beta tidak menunjukkan perbedaan yang bertahan setelah koreksi, sementara pada pita gamma hanya satu edge PLV (Fp2–P8) yang bertahan tanpa konfirmasi dari coherence maupun uji permutasi. Berdasarkan hasil tersebut, hypoconnectivity alpha frontocentral diusulkan sebagai kandidat biomarker EEG untuk membedakan penderita alkoholisme dari kontrol sehat, sedangkan reorganisasi topologi theta menjadi temuan pendukung yang masih terbatas pada satu metrik dan satu nilai densitas. Seluruh temuan bersifat asosiatif dan berkontribusi pada pengembangan penanda objektif berbasis EEG untuk AUD.

Alcohol use disorder (AUD) is a global health burden whose detection still relies largely on subjective questionnaires, creating a need for objective brain-based markers. Functional connectivity derived from EEG is a candidate for this role, yet prior findings remain inconsistent across frequency bands, regions, and the direction of change, partly because of reliance on a single metric and the influence of volume conduction. This study characterizes differences in brain functional connectivity between individuals with alcoholism and healthy controls across four frequency bands.
The analysis used the public UCI EEG dataset comprising 121 subjects, namely 76 with alcoholism and 45 controls, over 19 region-of-interest channels for the theta, alpha, beta, and gamma bands. Connectivity was computed with coherence and the phase locking value (PLV) as primary metrics and the weighted phase lag index (wPLI) as a volume-conduction-robust comparator. Group differences were tested at the edge level for all three metrics and at the network level through graph theory on weighted PLV networks thresholded at a density of 0.30. Statistical inference used the Mann–Whitney U test, Cliff’s delta effect size, false discovery rate (FDR) correction, permutation testing with 5,000 iterations, and Spearman correlation across metrics.
Edge-level differences concentrated in the alpha band over the frontocentral region, with 23 PLV edges and 22 coherence edges surviving FDR correction. Most reflected hypoconnectivity in the alcoholism group, with FC5, Cz, and C3 the most frequently involved channels. PLV and coherence were highly consistent (Spearman 0.978–0.984), whereas wPLI yielded no significant edges and therefore served as a conservative bound. At the network level, five theta-band graph metrics differed significantly, namely a higher clustering coefficient and global efficiency together with a shorter path length in the alcoholism group (Cliff’s delta 0.296–0.479). The beta band showed no differences that survived correction, while in the gamma band only a single PLV edge (Fp2–P8) survived, without confirmation from coherence or permutation testing. On the basis of these results, frontocentral alpha hypoconnectivity is proposed as a candidate EEG biomarker for distinguishing individuals with alcoholism from healthy controls, while theta network reorganization is a supporting finding still limited to a single metric and a single density value. All findings are associative and contribute toward the development of objective EEG-based markers for AUD.
