According to the Global Cancer Observatory 2022, Indonesia ranks eighth worldwide in breast cancer incidence among women. Despite advances in cancer treatment, adverse effects remain a major concern, highlighting the need for therapeutic approaches that are more effective, affordable, and associated with lower toxicity and cytotoxic resistance. Preliminary studies indicated that Arcangelisia flava (L.) Merr exhibits cytotoxic potential against MCF-7 cells, which represent a commonly used model of estrogen receptor-positive (ER+) breast cancer. CCND1, TP53, and BCL2 were selected as target genes because of their substantial expression in ER+ breast cancer and their potential as biomarkers for evaluating the effects of active A. flava fractions in vitro. Cytotoxicity was evaluated using the MTT assay to determine the most effective IC50 against MCF-7 cells through Bioassay-Guided Fractionation. Flow cytometry was performed to quantify apoptosis, while RT-qPCR was used to assess CCND1, BCL2, and TP53 expression. All assays compared cells treated with the active fraction with untreated controls. Natural compounds were identified using high-resolution mass spectrometry (HRMS). The active fraction exhibited cytotoxic activity against MCF-7 cells, with IC50 values of 42.49 ± 13.6 µg/mL at 24 h and 34.73 ± 1.85 µg/mL at 48 h. Apoptosis increased from 6.2% in controls to 7.2%, 9.3%, and 10.9% at 1/4IC50, 1/2IC50, and IC50, respectively. CCND1 expression decreased 1.80-fold at 24 h and 3.92-fold at 48 h, while BCL2 decreased 1.80-fold and 2.53-fold, respectively. Conversely, TP53 expression increased 1.70-fold and 3.29-fold. HRMS identified eight potential bioactive compounds: Citral, Isoferulic acid, 10-HDA, Scopoletin, Betaine, Formononetin, Palmitic Acid, and Genistein, which have been associated with apoptosis induction, MCF-7 growth inhibition, CCND1 and BCL2 suppression, and TP53 upregulation. The active fraction demonstrated cytotoxic and pro-apoptotic effects and modulated genes involved in cell-cycle regulation and apoptosis, supporting its potential for further development as a breast cancer therapeutic agent.
