Background: Diphtheria remains a public health concern in Indonesia, with recurrent outbreaks reported across several regions. Effective outbreak response depends not only on surveillance capacity but also on program implementation and risk communication. This study integrated these three components to provide a comprehensive understanding of diphtheria outbreak response.
Methods: This study employed an evaluative design using a mixed-methods approach consisting of three substudies. The first substudy evaluated the implementation of the diphtheria response program using a convergent mixed-methods. The second substudy evaluated the diphtheria surveillance system using a quantitative descriptive. The third substudy used a descriptive qualitative case series design to explore risk communication in three diphtheria outbreak areas: Yogyakarta City, Sleman Regency, and Bantul Regency.
Results: The surveillance system effectively supported early detection, notification, epidemiological investigation, and case response, although data analysis, feedback, and monitoring require further strengthening. Program implementation generally followed national guidelines, but its effectiveness was influenced by resource readiness, multisectoral coordination, and operational adaptability. Risk communication was initiated immediately after case notification, with strategies tailored to local contexts. Trust in health workers, stakeholder engagement, and adaptive communication enhanced community acceptance of interventions, whereas rumors, social stigma, and vaccine hesitancy remained key challenges.
Conclusion: Effective diphtheria outbreak response requires the integration of surveillance, adaptive program implementation, and risk communication that strengthens public trust and community acceptance. Strengthening these interconnected components is essential to improve preparedness and response to vaccine-preventable disease outbreaks.
