This study investigated the synthesis and characterization of mangosteen peel–derived carbon dots (CDots) (Garcinia mangostana L.) and evaluated their potential as photoacoustic imaging contrast agents and photothermal therapeutic agents. Photoacoustic imaging is a hybrid modality that combines spectroscopy-based optical contrast with acoustic signal detection generated by the thermoelastic effect, making it promising for medical diagnosis and therapy. This research aimed to evaluate the performance of naturally derived CDots as photoacoustic contrast agents and as candidates for photothermal therapy. Mangosteen peel was synthesized into CDots and characterized using UV–Vis spectroscopy, fluorescence spectroscopy, Fourier Transform Infrared (FTIR) spectroscopy, and High Resolution Transmission Electron Microscopy (HRTEM) to determine optical properties and particle morphology. The performance of CDots as contrast agents was evaluated by measuring the optical extinction coefficient and photothermal conversion efficiency, which were then applied in a simple photoacoustic imaging system using an 808 nm near-infrared (NIR) diode laser. CDots cytotoxicity was tested using Vero and HeLa cells, while photothermal therapy performance was evaluated through HeLa cell viability measurements after laser irradiation. The results showed that the synthesized CDots had an average diameter of 9 ± 2 nm, an absorption peak at 270 ± 1 nm, and maximum fluorescence emission in the 200–350 nm range. The optical extinction coefficient reached 0.004 mLmg-1cm-1 with a photothermal conversion efficiency of 36.19% in the NIR region and 0.149 mLmg-1cm-1 with an efficiency of 33.05% in the UV region. Cytotoxicity testing yielded a selectivity index of 1.212, indicating relative biocompatibility toward normal cells. Photothermal therapy reduced cell viability to 54.62% under UV irradiation, whereas viability remained 95.04% under NIR irradiation, demonstrating wavelength-dependent therapeutic effects. Two-way ANOVA results showed that the treatment had a significant effect on the optical and photothermal responses (p < 0>
