Background: emergency patients admitted to the hospitals emergency departement require prompt medical intervention and maybe of making informed decision based on the information provided by the attending physician. This situation presents an ethical dilemma for physicians. Physicians are ethically obligated to the principle of patient autonomy which recognizes an individuals right to self determination and is manifested throgh voluntarys, informed, non coercive and rational decision making. In the context of medical practice, informed consent is recognized as an expression of respect for the patient autonomy and personal integrity.

Objectives: This study to evaluate the respect for patient autonomy through the implementation of informed consent in emergency situations prior to surgical procedure, to assess surgeons level of knowledge and compliance regarding informed consent before performing medical procedures and to identify factors associated with surgeons knowledge and compliance with informed consent prior to surgical intervention.

Methods: The study adopted a mixed methods design with a sequential explanatory approach. Quantitative data were collected and analyzed in the first phase, followed by qualitative data collection and analysis in the second phase.

Research results: The characteristics of the responsdents showed that the majority were of productive age, had more than five years of clinical practice experience, worked in the public sector and were predominantly located in Medan. Most surgeons (65,4%) demonstrated a good level of knowledge regarding informed consent, and the level of surgeons compliance with informed consent implementation was categorized as compliant (94,2%). Spearman Rank analysis revealed a significant relationship between surgeons level of knowledge and their level of compliance. The qualitative indings indentified four main themes describing the implementation of informed consent in surgical emergency patients: (1) Physicians knowledge as the foundation of compliance, (2) Limitations of patient autonomy in emergency situation, (3) Informed consent as ethical communication in emergency settings, (4) Barriers and the need for strengthening hospital systems.

Conclusion: Respect for patient autonomy through the implementation of informed consent in emergency situations remains a fundamental principle for surgeons. Surgeons knowledge of informed consent as an expression of patient autonomy plays an essential role in surgical emergency care. Informed consent is not merely a signed authorization form but represents an ethical, legal and professional process. Compliance with informed consent implementation should be evaluated not only by the completeness of documentation but also by the quality of communication between physicians and patients or their families. A positive, significant and strong relationship was found between surgeons knowledge and compliance with informed consent implementation in surgical emergency settings. Factors affecting surgeons knowledge and compliance extend beyond individual characteristics and include situational factors and healthcare system related factors.
