Efek terapi kombinasi L-carnitine dan Ubiquinone terhadap dispersi QT pada penderita gagal ginjal kronik yang menjalani hemodialisis rutin

2005 | Tesis | PPDS I Ilmu Penyakit DalamLatar Belakang: Kadar L-carnitine dan ubiquinone pada penderita gagal ginjal kronik yang menjalani hemodialisis rutin menurun. Penurunan ini menyebabkan terjadinya stres oksidatif dan mengakibatkan ketidakstabilan membran sel. Keadaan ini menyebabkan kenaikan dispersi QT yang merupakan prediktor independen terjadinya aritmia ventrikel dan kematian jantung. Tujuan: Untuk mengetahui seberapa besar pengaruh pemberian kombinasi Lcarnitine dan Ubiquinone terhadap dispersi QT pada penderita gagal ginjal kronik yang menjalani hemodialisis rutin dibandingkan dengan plasebo. Bahan dan Cara Penelitian: Enam puluh dua penderita gagal ginjal kronik yang menjalani hemodialisis rutin di Instalasi Dialisis RS Sardjito Yogyakarta ikut dalam penelitian klinik acak buta ganda ini. Subyek dibagi menjadi 2 kelompok. Kelompok I (33 subyek) mendapat kombinasi L-carnitine 2000 mg dan Ubiquinone 120 mg per hari peroral selama 8 minggu dan kelompok II (29 subyek) mendapat plasebo. Dilakukan analisis dispersi QT dan dispersi QTc sebelum dan sesudah perlakuan. Hasil: Rerata dispersi QT sesudah perlakuan pada kelompok terapi menurun secara bermakna dibandingkan dengan plasebo (56,99 ± 17,59 menjadi 43,89 ± 15,45 milidetik vs 50,26 ± 11,82 menjadi 52,67 ± 13,08 milidetik, p=0,020). Rerata dispersi QTc pada kelompok terapi juga berbeda secara bermakna dibandingkan dengan kelompok plasebo (65,97 ± 22,24 menjadi 51,69 ± 18,73 milidetik vs 58,48 ± 15,70 menjadi 61,57 ± 17,51 milidetik, p=0,037). Kesimpulan: Kombinasi L-carnitine 2000 mg dengan ubiquinone 120 mg per hari per oral selama 8 minggu pada penderita gagal ginjal kronik yang menjalani hemodialisis rutin dapat menurunkan rerata dispersi QT dan dispersi QTc lebih besar secara bermakna dibandingkan dengan plasebo.
Background: L-carnitine and ubiquinone level in chronic kidney disease patients on routine hemodialysis were decreased which lead to oxidative stress and cause cell membrane instability. This lead to increased of QT dispersion, an independent predictor of ventricular arrhythmia and cardiac death. Objectives: to determine how much the effect of L-carnitine and ubiquinone combination on QT dispersion in chronic kidney disease patients on routine hemodialysis compared with placebo. Methods: Sixty-two chronic kidney disease patients on routine hemodialysis were included in this randomized-double blind placebo controlled trial. Group I (33 subjects) received combination of 2000 mg L-carnitine and 120 mg ubiquinone oraly for 8 weeks, and group II (29 subjects) received placebo. QT dispersion measured in the 12-lead standard electrocardiogram before and after treatment was analyzed. Results: Mean of QT dispersion in group I was decreased (from 56,99 ± 17,59 to 43,89 ± 15,45 miliseconds) while in the group II was increased (from 50,26 ± 11,82 to 52,67 ± 13,08 miliseconds). There was a significant different of mean of QT dispersion between the two groups (p=0,020). Mean of QTc dispersion in group I was decreased (from 65,97 ± 22,24 to 51,69 ± 18,73 miliseconds) while in the group II was increased (from 58,48 ± 15,70 to 61,57 ± 17,51 miliseconds). There was a significant different of mean of QTc dispersion between the two groups (p=0,037). Conclusions: Combination of L-carnitine and ubiquinone decreased QT and QTc dispersion better than placebo.
