Background: Digital health transformation has accelerated the implementation of teledentistry as an innovative remote dental care service. However, its successful adoption is highly dependent on the behavioral intention of dentists as service providers.

Objective: This study aimed to analyze the determinant factors influencing dentists' intention to utilize teledentistry services using the Unified Theory of Acceptance and Use of Technology (UTAUT) model in the Special Region of Yogyakarta (DIY).

Method: This quantitative observational analytic study employed a cross-sectional design, involving 90 dentists in DIY. Data were collected using a structured questionnaire and analyzed using Spearman correlation and Structural Equation Modeling-Partial Least Square (SEM-PLS).

Results: Facilitating Conditions emerged as the strongest predictor of dentists' intention to use teledentistry (path coefficient = 0.301; p = 0.001), followed by Effort Expectancy (path coefficient = 0.282; p = 0.006) and Social Influence (path coefficient = 0.225; p = 0.010). Performance Expectancy showed no significant effect (path coefficient = 0.112; p = 0.368). The UTAUT model explained 60.3% of the variance in dentists' behavioral intention (R² = 0.603), with a predictive relevance value (Q²) of 0.555. Social Influence demonstrated the highest correlation strength (r = 0.468).

Conclusion: The availability of supporting infrastructure, ease of technology use, and professional-social environmental support are the primary determinants of teledentistry adoption intention in DIY. Strengthening digital infrastructure, technical training, and institutional as well as professional organization support are essential to promote sustainable teledentistry adoption.
