Background: Preterm birth is a major clinical problem associated with perinatal morbidity and mortality. Aim: The aim of the study is to identify risk factors associated with preterm birth in Purworejo District. Methods: A cohort-nested study was conducted and the data was obtained from a secondary data. Results: A total of 343 cases were identified as preterm births out of 2098 pregnant women were involved in this study. Backward regression analysis was done. Several significant risk associations between preterm birth and the following risk factors were as follows: maternal age RR (1.754), economical status RR (1.415), maternal education level RR (1.721), parity RR (1.279), and baby's birth weight RR (14.387). Conclusion: The main determinant of preterm birth is baby's birth weight and there was no factor associated with it.
