Eksplorasi Rendahnya Cakupan Penemuan Kasus Tuberkulosis Di Kabupaten Bangka Barat Provinsi Bangka Belitung

Latar belakang : upaya dalam penanggulangan tuberkulosis adalah dengan meningkatkan cakupan penemuan tuberkulosis nasional. Kabupaten bangka Barat memiliki cakupan penemuan kasus tuberkulosis sebesar 27 % sebagai kabupaten terendah di Bangka Belitung. Diperlukan analisis yang mendalam terkait faktor penyebab rendahnya cakupan penemuan kasus tuberkulosis dalam sisi skrining dan diagnosis tuberkulosis yaitu faktor tenaga kesehatan, faktor akses pelayanan kesehatan dan peranan kader.
Tujuan : mengeksplorasi dan memahami permasalahan rendahnya penemuan kasus tuberkulosis di Kabupaten Bangka Barat berkaitan dengan skrining dan diagnosis tuberkulosis.
Metode : penelitian ini adalah penelitian kualitatif dengan jenis studi kasus dengan mengeksplorasi data SITB dan dokumen, wawancara semistruktur tenaga kesehatan dan kader yang berperan dalam program tuberkulosis. Total responden 28 orang yang dipilih purposif dari 3 puskesmas cakupan penemuan kasus tuberkulosis tertinggi dan 3 puskesmas cakupan penemuan terendah berdasarkan data Dinas Kesehatan Kabupaten Bangka Barat tahun 2024.
Hasil : tingginya beban tenaga kesehatan disebabkan oleh kurangnya SDM, banyaknya program dan aplikasi kesehatan yang mempengaruhi cakupan penemuan kasus tuberkulosis, tidak terdapat permasalahan dari sisi pendidikan, pengetahuan dan lama kerja tenaga kesehatan. Dari sisi akses pelayanan, pemeriksaan menggunakan TCM dengan keterbatasan katrid dan kualitas spesimen yang kurang optimal. Peranan kader belum optimal khususnya dalam skrining dan investigasi kontak serta rendahnya kepatuhan masyarakat. Stigma sosial budaya. Investigasi kontak belum memenuhi standar namun berkontribusi dalam penemuan kasus tuberkulosis meskipun persentasenya rendah.
Kesimpulan : rendahnya cakupan penemuan tuberkulosis di Kabupaten Bangka Barat disebabkan oleh tingginya beban tenaga kesehatan, adanya keterbatasan katrid TCM serta kualitas spesimen yang kurang baik dari sisi akses, rendahnya kesadaran masyarakat dan kuatnya stigma sosial budaya serta peranan kader yang kurang optimal.
Background : one of the key strategies for tuberkulosis control is to increase the national tuberculosis case finding coverage. West Bangka Regency has a tuberculosis case finding coverage of 27 %, the lowest among all regencies ini Bangka Belitung Province. An in depth analysis is therefore needed to identify the factor contributing to the low tuberculosis case finding coverage, particularly those related to tuberculosis screening and diagnosis, including healthcare workforce factors, access to healthcare services, and the role of cadres.
Objective : to explore and understand the factors contributing to the low tuberculosis case finding ini West Bangka Regency, particularly those associated with tuberculosis screening and diagnosis.
Methods : this qualitative study employed a case study approach. Data were collected through the exploration of the Integrated Tuberculosis Information System (SITB) database and document. Interviews with the healthcare workers and cadres involved in the tuberculosis program. 28 participants were selected from three primary health centers (PHCs) with the highest tuberculosis case finding coverage and three PHCs with the lowest tuberculosis case finding coverage, based on the data 2024 data from West Bangka District Health Office.
Result : the high workload among healthcare workers was primarily attributed to shortages of human resources and the large number of health program which affected tuberculosis case finding coverage. No major issues were identified regarding the educational background, knowledge or years of services of healthcare workers. From the healthcare access perspective, tuberculosis diagnosis using the Xpert MTB/RIF system with limited catridge availability and suboptimal specimen quality. The role of cadres had not been fully optimized. Low community compliance and persistent sociocultural stigma hindered tuberculosis case finding effort.
Conclusion : the low tuberculosis case finding coverage in West Bangka Regency is primariy attributable to the heavy workload healthcare workers, limited availability of Xpert MTB/RIF catridges and suboptimal specimen quality, low public awareness, strong sociocultural stigma, and the suboptimal onvolvement of community health volunteeers in tuberculosis control activities.
