Johannes Kupang periode 2022–2025.

Total sampel berjumlah 29 pasien.

Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection, with ICU mortality rates reaching 42% globally and 69% in Indonesia. The use of empirical antibiotics without evaluation contributes to antimicrobial resistance. In Indonesia, quantitative antibiotic-use evaluations in ICUs remain limited, particularly in Eastern Indonesia. This study aimed to assess patient characteristics, antibiotic utilization patterns, and quantitative antibiotic consumption using the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) method, Drug Utilization 90% (DU 90%), and the Prescribed Daily Dose/Defined Daily Dose (PDD/DDD) ratio among sepsis patients in the ICU of Prof. Dr. W.Z. Johannes Regional General Hospital, Kupang, during 2022-2025.

This retrospective descriptive observational study included all adult patients (18 years of age or older) diagnosed with sepsis who received intravenous antibiotics (ATC code J01) and had complete medical records. Patients with a length of stay of less than 24 hours were excluded. A total of 29 patients met the study criteria. Data analysis consisted of DDD/100 bed-days calculation based on WHO standards, DU 90% segment identification, longitudinal trend analysis, and PDD/DDD ratio calculation.

Ceftriaxone dominated antibiotic consumption throughout 2022-2025, with  DDD/100 bed-days ranging from 48.21 to 94.12 and DU 90% proportions ranging  from 48.25% to 76.19%. Increased ceftriaxone consumption in 2023 was associated with the PPRA Team’s meropenem restriction policy in response to excessive use. Ceftriaxone and levofloxacin consistently appeared in the DU 90% segment. The PDD/DDD ratio was consistently 1.50 for levofloxacin, varied from 0.86 to 1.53 for ceftriaxone and from 0.85 to 1.14 for meropenem. The dominance of the Watch group in the AWaRe classification (71.60%-97.62%) indicates a potential risk of resistant bacterial development without strict monitoring. These findings confirm ceftriaxone as the primary empirical antibiotic for sepsis patients in the ICU and  highlight the need to strengthen Antimicrobial Stewardship programs to prevent antimicrobial resistance.
