Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by skin barrier dysfunction and a predominance of T helper 2 (Th2)-mediated immune responses. This study aimed to evaluate the effects of jackfruit leaf (Artocarpus heterophyllus) extract cream on the expression of CD4, IL-4, and CK-10 in 30 male Balb/c mice induced with 2,4-dinitrochlorobenzene (DNCB). The study employed a laboratory experimental design consisting of five treatment groups, with six mice in each group. The groups included a normal control (without DNCB induction), a negative control induced with DNCB and treated with cream base without extract, a positive control induced with DNCB and treated with desoximetasone ointment, and two treatment groups induced with DNCB and treated with jackfruit leaf extract cream at concentrations of 3% and 5%, respectively. Mice were induced with 1% DNCB during the sensitization phase (days 1, 4, and 7) and 0.5% DNCB during the challenge phase (days 14, 17, 19, 22, 24, 27, and 29), administered at a dose of 200 ?L on the dorsal skin and 20 ?L on the posterior surface of the right ear. The treatment period was terminated on day 30, followed by the collection of dorsal skin and ear tissue samples. Skin samples were processed histologically and subjected to immunohistochemical staining to evaluate CD4, IL-4, and CK-10 expression using the Immunoreactivity Score (IRS) assessment method. The results showed that DNCB induction in the negative control group increased CD4 and IL-4 expression while decreasing CK-10 expression compared with the normal control group, indicating inflammatory activation and impaired epidermal differentiation. Administration of jackfruit leaf extract cream at concentrations of 3% and 5% significantly improved atopic dermatitis lesions, as evidenced by decreased CD4 and IL-4 expression and increased CK-10 expression in the epidermis compared with the negative control group. These findings indicate that jackfruit leaf extract cream possesses anti-atopic potential through immunomodulatory mechanisms characterized by suppression of inflammatory activity and enhancement of CK-10 expression associated with the restoration of epidermal integrity.
