This study developed an optical fiber-based photoacoustic imaging system as an approach to improve system portability while evaluating the influence of multimode optical propagation on the formation of photoacoustic image intensity distributions. The developed system consisted of a 532 nm diode laser, a multimode optical fiber serving as the optical waveguide, a condenser microphone as the acoustic detector, and a two-dimensional scanning mechanism. Experiments were conducted using homogeneous phantom samples and biological meat samples subjected to various oil immersion treatments. System evaluation included laser power characterization, acoustic response analysis, image structural similarity assessment (SSIM), contrast-to-noise ratio (CNR), and texture analysis using region of interest (ROI)-based GLCM entropy. The characterization results demonstrated that the optical fiber-based system was able to maintain an average optical power comparable to the conventional direct laser system, namely 74.37 mW compared to 75.08 mW, while exhibiting improved stability indicated by the reduction of power standard deviation from 3.84 mW to 0.76 mW and RMS fluctuation from 5.12% to 1.02%. The generated acoustic response was also successfully preserved, indicating that the optical fiber configuration can be effectively utilized as an optical propagation configuration in the developed photoacoustic imaging system. In homogeneous phantom imaging, the FASO system preserved object structures comparable to the conventional FAD system with high SSIM values (0.88–0.90), while exhibiting more dominant local granular patterns. Basic image statistical analysis showed that the mean intensity, CNR, and SNR of both systems were relatively comparable, indicating that the observed differences in image characteristics could not be directly explained through global statistical parameters alone. ROI-based GLCM entropy analysis revealed a clearer distinction, where entropy values in FAD images ranged from 7.83 to 8.19, whereas FASO images increased to 8.78–9.15. This increase in texture complexity indicates that the optical fiber-based system produces higher local intensity heterogeneity compared to the conventional system. The difference in image characteristics between the FAD and FASO systems is explained by the multimode interference phenomenon within the optical fiber. Coherent superposition among guided modes generates modal interference that modulates the optical fluence distribution spatially. The modulated optical fluence subsequently alters the initial photoacoustic pressure distribution and the measured acoustic amplitude at each scanning point, resulting in the formation of local granular patterns in the reconstructed images. In biological sample imaging, ROI-based GLCM entropy values increased systematically from approximately 7.86 in the control sample to approximately 8.05 for coconut oil treatment, 8.33 for sesame oil treatment, 8.89 for waste cooking oil treatment, and reached approximately 8.96 for pork oil treatment. This increase indicates enhanced optical heterogeneity within the tissue caused by variations in optical absorption and scattering distributions. In the optical fiber-based system, fluence heterogeneity induced by multimode interference enhanced the system sensitivity toward tissue microstructural variations, such that the observed image granularity not only represented system characteristics but also contained diagnostic information associated with the biological condition of the sample. The results demonstrate that the integration of multimode optical fibers not only improves the stability and portability of the photoacoustic imaging system, but also modifies the optical fluence distribution involved in photoacoustic signal formation, thereby enriching image texture characteristics and enhancing sensitivity toward biological structural variations.
