Diabetes mellitus (DM) is a chronic metabolic disorder whose prevalence in Indonesia is projected to reach 28.6 million cases by 2045. This condition occurs not only in humans but has also been reported in animals, particularly dogs and cats. One of the major complications of DM is diabetic foot ulcer (DFU), which remains the leading cause of non-traumatic lower-limb amputation. This study aimed to evaluate the wound-healing activity of extracellular vesicles (eV) derived from the secretome of equine dental pulp stem cells (eDPSCs) as an alternative therapy for diabetic wounds in a rat model, and to determine the gel formulation that provides the most effective healing activity. Type 2 diabetes was induced in Wistar rats (Rattus norvegicus), which were then treated with eV-enriched gel formulations at concentrations of 10%, 20%, and 30%. Wound closure percentage over a 14-day period was analyzed using SPSS v25.0, employing the Kruskal–Wallis test followed by Mann–Whitney post-hoc analysis. The results demonstrated that the 30% DPSC-eV gel significantly accelerated diabetic wound healing (p<0.05). Histopathological examination further showed that the 30% eV gel promoted faster tissue regeneration, characterized by more complete structural integrity and advanced maturation of the wound bed. These findings highlight the therapeutic potential of equine DPSC-derived extracellular vesicle gel as an effective alternative strategy for managing diabetic wounds and may offer a promising avenue for future translational studies in both veterinary and human medicine.
