Blood clams (Anadara granosa) are one of the most popular types of clams consumed from Indonesia’s fisheries. Their shell waste can be utilized to produce hydroxyapatite (HA), which is useful for health, especially for dental problems. This study aims to make hydroxyapatite (HA) composite gel with added betel leaf as antibacterial, in order to be useful as a remineralisation agent of teeth. Hydroxyapatite based on blood shells was synthesised using the precipitation method, then the results were used to make gel composites. Gel base was prepared using Na-CMC, Guar-Gum, and glycerin. Gel composites were synthesised from each HA with concentrations of 5, 10, 15, and 20 wt?ded with 20 wt?tel leaf extract. Fabricated gel composites were characterised using Scanning Electron Microscope (SEM), Fourier Transform Infrared (FTIR) and X-Ray Diffractometer (XRD) and conducted in vitro tests through antibacterial tests on the inhibition zone of Streptococcus mutans and Streptococcus sanguinis bacteria. Antibacterial test aims to determine the inhibition of bacteria against bacteria that cause dental caries. Results of the physicochemical characterization of HA showed a surface particle size distribution of (190 ± 1) nm with a Ca/P ratio of 1.62, which is close to the ideal value of 1.67 for HA, a crystal size of (50 ± 6) nm, and confirmed HA functional groups. Physicochemical results of the gel composite showed that crystallite size was maintained at the nanometer scale, namely (31–47) nm, with HA functional groups present, and the nano-HA particles appeared to be more evenly distributed with uniform particle sizes at higher concentrations but became agglomerated. Remineralization test results confirmed that HA concentration is important for supplying calcium and phosphate, but it does not always correlate directly with an increase in tooth hardness. The results of the antibacterial test showed an increase in antibacterial activity with increasing HA concentrations and addition of betel leaf extract, confirming that gel composite is safe to use and has the potential to repair dental enamel caries through remineralization process.
