Identification of Clinicopathological Characteristics at Diagnosis Associated with Lung Metastasis in Breast Cancer

Kasus kanker payudara metastatik diketahui memiliki prognosis yang buruk dengan angka ketahanan hidup (overall survival) hanya sebesar 31%. Salah satu lokasi metastasis jauh yang paling sering pada kanker payudara adalah paru-paru. Penelitian ini merupakan studi kohort observasional retrospektif yang bertujuan untuk mengidentifikasi karakteristik klinikopatologis pada saat diagnosis yang berhubungan dengan terjadinya metastasis paru pada pasien kanker payudara. Penelitian ini menggunakan data rekam medis elektronik dari total 1003 pasien kanker payudara yang didiagnosis di Divisi Onkologi Bedah, RSUP dr. Sardjito pada periode 2014 - 2019. Variabel klinis yang dianalisis meliputi ukuran tumor, stadium tumor, tipe histopatologi, derajat histopatologi, subtipe molekuler, status menopause, status reseptor hormon dan reseptor faktor pertumbuhan, invasi limfovaskular, status kelenjar getah bening, serta variabel sosiodemografi. Seluruh variabel tersebut dianalisis untuk mengetahui hubungannya dengan kejadian metastasis paru melalui analisis bivariat dan multivariat. Kejadian metastasis paru ditentukan berdasarkan identifikasi tanda-tanda klinis yang kemudian dikonfirmasi menggunakan pemeriksaan pencitraan (imaging) yang dilakukan setelah penyelesaian terapi, dengan masa tindak lanjut (follow-up) minimal 6 bulan sejak diagnosis awal.

Cases of metastatic breast cancer are known to have a limited prognosis with an overall survival rate of only 31%. One of the most common sites for distant metastasis in breast cancer are the lungs. This study is a retrospective observational cohort study aiming to identify the clinicopathological characteristics at the time of diagnosis that are associated with lung metastasis in breast cancer patients. This study used electronic medical record data of a total of 1003 breast cancer patients diagnosed at the Division of Surgical Oncology, RSUP dr. Sardjito between 2014 - 2019 from which clinical variables such as tumor size, tumor stage, histological type, histological grade, molecular subtype, menopause, hormone and growth receptor status, lymphovascular invasion, nodal status as well as sociodemographic variables are collected and analyzed for their association with lung metastasis occurrence through bivariate and multivariate analysis. Incidence of lung metastasis was diagnosed through identification of clinical signs which were then confirmed using imaging tests done after completion of treatment at a minimum follow up of 6 months after initial diagnosis.
