Hubungan Partisipasi Peserta Prolanis Dalam Konsultasi Medis Bulanan Terhadap Pengendalian Diabetes Mellitus Tipe 2 Di Fasilitas Kesehatan Tingkat Pertama Kabupaten Indragiri Hulu

Latar Belakang: Diabetes
Mellitus (DM) tipe 2 merupakan penyakit kronis dengan prevalensi meningkat di
Indonesia, dari 6,9% (2013) menjadi 10,9-11,7% (2023). BPJS Kesehatan
mengembangkan Program Pengelolaan Penyakit Kronis (PROLANIS) sebagai strategi
pengendalian DM dengan konsultasi medis bulanan sebagai komponen inti untuk
monitoring klinis, penyesuaian terapi, dan edukasi self-management. Namun,
efektivitas partisipasi konsultasi medis bulanan terhadap outcome pengendalian
glikemik masih perlu evaluasi mendalam di tingkat Fasilitas Kesehatan Tingkat
Pertama (FKTP).
Tujuan: Menganalisis
hubungan partisipasi peserta PROLANIS dalam konsultasi medis bulanan terhadap
pengendalian DM tipe 2 di FKTP Kabupaten Indragiri Hulu yang diukur melalui
perubahan kadar Hemoglobin A1c (HbA1c).
Metode: Penelitian cross-sectional dengan pendekatan
kuantitatif menggunakan data sekunder dari sistem P-Care BPJS Kesehatan dan
rekam medis FKTP periode Januari-Desember 2024. Sampel sebanyak 370 peserta
PROLANIS DM tipe 2 dari populasi 2.349 peserta. Variabel independen adalah
partisipasi konsultasi medis bulanan (aktif ?9 kali kunjungan, tidak aktif

menggunakan uji Chi-Square dan Odds Ratio dengan ?=0,05.
Hasil: Sebagian besar peserta (71,6%) memiliki
partisipasi aktif. Terdapat hubungan bermakna antara partisipasi konsultasi
medis bulanan dengan pengendalian DM tipe 2 (p=0,013; OR=1,844;
95%CI=1,168–2,912). Peserta aktif memiliki peluang 1,8 kali lebih besar
mencapai outcome baik dibandingkan yang tidak aktif. Sebanyak 62,6% peserta
aktif mencapai outcome baik, sementara 47,6% peserta tidak aktif. Analisis ROC mengidentifikasi ambang batas optimal
kunjungan pada ?9 kali per tahun (AUC=0,552; sensitivitas 76,9%; spesifisitas
35,7%).
Background: Type 2 Diabetes
Mellitus (DM) is a chronic disease with increasing prevalence in Indonesia,
from 6.9% (2013) to 10.9-11.7% (2023). BPJS Kesehatan developed the Chronic
Disease Management Program (PROLANIS) as a DM control strategy with monthly
medical consultation as a core component for clinical monitoring, therapy
adjustment, and self-management education. However, the effectiveness of
monthly medical consultation participation on glycemic control outcomes
requires in-depth evaluation at Primary Healthcare Facilities (FKTP) level.
Purpose: To analyze the relationship between PROLANIS participants'
participation in monthly medical consultations and type 2 DM control at FKTP
Indragiri Hulu Regency, measured through changes in Hemoglobin A1c (HbA1c)
levels.
Methods: Cross-sectional
study with quantitative approach using secondary data from BPJS Kesehatan
P-Care system and FKTP medical records for January-December 2024 period. Sample
of 370 type 2 DM PROLANIS participants from a population of 2,349 participants.
Independent variable was monthly medical consultation participation (active ?9
visits, inactive <9>?=0.05.
Results: Majority of participants (71.6%) had active
participation. A significant association was identified between monthly medical
consultation participation and type 2 DM control (p=0.013; OR=1.844;
95%CI=1.168-2.912). Active participants had 1.8 times greater odds of achieving
good outcomes compared to inactive participants. A total of 62.6% of active
participants achieved good outcomes, while 47.6% of inactive participants did
so. ROC analysis identified an optimal visit threshold of ?9 visits per year
(AUC=0.552; sensitivity 76.9%; specificity 35.7%), providing an empirical basis
for realistic yet clinically meaningful program targets.
Conclusion: Active participation in PROLANIS monthly medical consultations increases
the likelihood of achieving good glycemic control in type 2 DM patients at FKTP.
