Efek Antioksidan Neoagarooligosakarida terhadap Stres Oksidatif dan Ekspresi Gen NOS2 pada Mencit (Mus musculus Linnaeus, 1758) Hiperglikemia

Hiperglikemia merupakan ciri utama dari diabetes melitus yang dapat memicu stres oksidatif dan nitrosatif akibat produksi berlebih reactive oxygen species (ROS) dan disregulasi pensinyalan nitric oxide (NO). Kondisi ini meningkatkan kadar malondialdehyde (MDA) melalui peroksidasi lipid serta memicu overekspresi inflamasi inducible nitric oxide synthase (NOS2). Penelitian ini mengevaluasi efek neoagarooligosakarida (NAOS) dari alga merah yang memiliki sifat antioksidan dan antiinflamasi namun belum diteliti menyeluruh secara in vivo dengan menggunakan mencit (Mus musculus Linnaeus, 1758) yang diinduksi hiperglikemia terhadap parameter tersebut. Hiperglikemia pada 18 ekor mencit BALB/c jantan diinduksi menggunakan high fat diet (14 hari) dan injeksi streptozotocin (55 mg/kgBB). Mencit dibagi menjadi enam kelompok, kontrol, kontrol negatif, kontrol positif (metformin 50 mg/kgBB), serta NAOS dosis 100, 200, dan 400 mg/kgBB yang diberikan melalui oral gavage selama 28 hari. Parameter yang diuji meliputi glukosa darah dengan GOD-PAP, MDA plasma dengan TBARS, serta NOx plasma dan hepar dengan Griess assay. Ekspresi gen NOS2 hepar dianalisis menggunakan RT-qPCR (2^-??CT) dengan gen referensi GAPDH. Hasil menunjukkan bahwa NAOS secara signifikan menurunkan kadar glukosa darah, MDA plasma, NOx plasma dan hepar, dan ekspresi NOS2 hepar dibandingkan dengan kontrol negatif. NAOS dosis 200 menunjukkan efek terbaik pada kadar glukosa dan NOx plasma dan hepar, sedangkan NAOS dosis 400 menghasilkan nilai fold change NOS2 terendah. Perbedaan ini sejalan dengan adanya regulasi NOS2 di tingkat pasca-transkripsi dan pasca-translasi yang bertingkat. Metformin mampu menurunkan kadar MDA plasma dan NOx plasma, tidak berhasil menurunkan kadar NOx hepar atau ekspresi NOS2 secara signifikan. Hal ini mencerminkan ketidakmampuan metformin dalam mengatasi mikrolingkungan inflamasi spesifik di hepar yang dipicu oleh LPS portal dan lipotoksistas. Temuan ini menunjukkan bahwa NAOS memberikan efek antioksidan dan antiinflamasi yang bersifat hepatoprotektif melalui mekanisme prebiotik yang dimediasi oleh usus. Mekanisme ini melibatkan supresi konvergen aktivitas NF-?B yang bergantung pada SCFA melalui aktivasi HDAC/Nrf2, supresi IKK yang dimediasi GPR43, serta aktivasi AMPK. Profil mekanistik ini lebih luas dibandingkan dengan metformin karena menargetkan langsung pemicu inflamasi upstream di hepar.
Hyperglycemia, the defining feature of diabetes mellitus that drives oxidative and nitrosative stress through excessive reactive oxygen species (ROS) production and dysregulated nitric oxide (NO) signaling, promoting lipid peroxidation reflected by elevated malondialdehyde (MDA) and inflammatory overexpression of inducible nitric oxide synthase (NOS2). Neoagarooligosaccharides (NAOS) are bioactive oligosaccharides derived from red algae agarose that have been reported to have antioxidant and anti-inflammatory properties, yet their effects on oxidative and nitrosative stress and hepatic NOS2 expression under hyperglycemic conditions (in vivo) have not yet been thoroughly investigated. This study evaluated the effects of NAOS on fasting blood glucose, plasma MDA, plasma and hepatic NOx levels, and hepatic NOS2 gene expression in hyperglycemia induced mice (Mus musculus Linnaeus, 1758). Hyperglycemia was induced by 14-day high-fat diet followed by streptozotocin injection (55 mg/kgBW). Eighteen male BALB/c mice were divided into six groups, control, negative control, positive control (metformin 50 mg/kgBW), and NAOS at 100, 200, and 400 mg/kgBW, all administered by oral gavage for 28 days. Glucose was measured by GOD-PAP spectrophotometry, MDA by TBARS assay, and NOx by Griess assay in plasma and hepatic tissue. NOS2 gene expression was analyzed by RT-qPCR using the 2^-??CT method with GAPDH as reference gene. NAOS significantly reduced fasting blood glucose, plasma MDA, plasma NOx, hepatic NOx, and hepatic NOS2 expression compared to the negative control, with NAOS 200 showing optimal glycemic and hepatic NOx reduction while NAOS 400 produced the lowest NOS2 fold change, a discrepancy consistent with multilevel post-transcriptional and post-translational NOS2 regulation. Metformin reduced plasma MDA and plasma NOx but failed to significantly reduce hepatic NOx or NOS2 expression, reflecting its inability to resolve the hepatic specific inflammatory microenvironment driven by portal LPS and lipotoxicity. These findings demonstrate that NAOS exerts hepatoprotective antioxidant and anti-inflammatory effects through a gut mediated prebiotic mechanism involving SCFA dependent convergent suppression of NF-?B activity through HDAC/Nrf2 activation, GPR43-mediated IKK suppression, and AMPK activation, a mechanistically broader profile than metformin that directly targets upstream hepatic inflammatory triggers.
