Parameter diagnostik meliputi sensitivitas, spesifisitas, nilai ramal positif (NRP), nilai ramal negatif (NRN), likelihood ratio positive (LRP), likelihood ratio negative (LRN), akurasi, area under the curve (AUC), kappa Cohen, dan uji McNemar.
Background: Complex coronary lesions in chronic coronary syndrome (CCS) are associated with greater procedural difficulty, need for dedicated devices, and staged intervention. Invasive coronary angiography (ICA) remains the reference standard for evaluating coronary anatomy and lesion complexity. Coronary computed tomography angiography (CCTA) may provide non-invasive pre-procedural anatomic information to support early stratification and procedural planning. Local data on the diagnostic accuracy of CCTA for identifying complex coronary lesions in CCS remain limited.
Objective: To evaluate the diagnostic accuracy of CCTA for identifying complex coronary lesions according to ACC/AHA subtype C classification, using ICA as the reference standard, in CCS patients at Dr. Sardjito General Hospital.
Methods: This cross-sectional diagnostic accuracy study included suspected CCS patients aged 18–75 years who underwent CCTA before ICA and met the study criteria. Subjects were enrolled by consecutive sampling. CCTA and ICA findings were reassessed to determine the presence of complex coronary lesions at patient and vessel levels. Diagnostic parameters included sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR+), negative likelihood ratio (LR?), accuracy, area under the curve (AUC), Cohen’s kappa, and McNemar test.
Results: A total of 61 patients were analyzed, with a mean age of 61±9 years and male predominance (41 patients; 67.2%). Based on ICA, complex coronary lesions were found in 35 patients (57.4%). At patient level, CCTA showed sensitivity 77.1%, specificity 53.8%, PPV 69.2%, NPV 63.6%, LR+ 1.67, LR? 0.42, accuracy 67.2%, kappa 0.32, and AUC 0.65. At vessel level, CCTA showed sensitivity 59.7%, specificity 83.0%, PPV 54.4%, NPV 85.8%, LR+ 3.5, LR? 0.49, accuracy 77.1%, kappa 0.41, and AUC 0.71. McNemar test showed bidirectional discordance between CCTA and ICA.
Conclusion: CCTA demonstrated moderate diagnostic performance for identifying complex coronary lesions at patient level, with sensitivity reaching the predefined study target. At vessel level, performance was characterized by higher specificity and NPV. These findings support CCTA as an early stratification and pre-procedural planning tool in CCS patients with suspected complex coronary lesions.
