Introduction and Objective: Hypospadias is a congenital anomaly in males requiring surgical correction, with chordectomy-urethroplasty as the standard procedure for cases with penile curvature. The choice of suture materialstructure (monofilament vs. multifilament) may influence healing and clinical outcomes, but there is no consensus on which type is more optimal in the context of the Indonesian population.

Methods: This retrospective cohort study was conducted at Dr. Sardjito General Hospital, Yogyakarta, involving 162 pediatric male patients (age 6 months–18 years) who underwent primary hypospadias surgery (chordectomy-urethroplasty) between 2020–2025. Patients were divided into monofilament (n=72) and multifilament (n=90) suture groups. Demographic data, hypospadias type, length of stay (LOS), and early (?14 days) and delayed (?6 months) complications were collected from medical records. Statistical analysis used the Mann-Whitney and Chi-square tests with a significance level of p<0>

Results: The median patient age was 5 years (IQR 2–11) with no significant difference between groups. Thedistribution of hypospadias types (glanular, distal, proximal) was also similar. The monofilament group had a significantly longer LOS (7 days vs. 5 days; p<0 p=0.21) p=0.77).>

Conclusion: There is no significant difference in clinical outcomes between the use of monofilament and multifilament sutures in hypospadias surgery with chordectomy- urethroplasty. The suture structure type cannot be used as the sole determinant of postoperative complications. Suture selection should consider patient characteristics, surgical preference, and the advantages and disadvantages of each suture type.
