ANALISIS FAKTOR DEMOGRAFI DAN DIAGNOSIS DALAM PENETAPAN KOEFISIEN TARIF KAPITASI DI FASILITAS KESEHATAN TINGKAT PERTAMA

Latar belakang: Penetapan tarif kapitasi di Fasilitas Kesehatan Tingkat Pertama (FKTP) dalam Program Jaminan Kesehatan Nasional (JKN) saat ini masih didasarkan pada faktor demografi berupa usia dan jenis kelamin sebagaimana diatur dalam Peraturan Menteri Kesehatan Nomor 3 Tahun 2023. Namun, variasi diagnosis penyakit berpotensi memengaruhi kebutuhan layanan kesehatan dan belum sepenuhnya terakomodasi dalam penyesuaian koefisien kapitasi.
Tujuan: Penelitian ini bertujuan untuk menganalisis kesesuaian koefisien kapitasi pada Peraturan Menteri Kesehatan Nomor 3 Tahun 2023 dengan pola utilisasi layanan di FKTP berdasarkan faktor demografi (usia dan jenis kelamin) serta diagnosis diabetes melitus dan hipertensi.
Metode: Penelitian ini merupakan studi kuantitatif dengan desain cross-sectional menggunakan data sekunder BPJS Kesehatan tahun 2023–2024. Sampel penelitian mencakup 255 FKTP dari total 341 FKTP di Kota Semarang. analisis menggunakan regresi Negative Binomial terhadap total kunjungan FKTP dengan memasukkan faktor demografi, diagnosis, jenis FKTP, dan tahun layanan, lalu koefisien empiris dihitung dari angka kontak untuk dibandingkan dengan Peraturan Menteri Kesehatan Nomor 3 Tahun 2023.
Hasil : Hasil penelitian menunjukkan bahwa kelompok usia lanjut memiliki tingkat kunjungan yang lebih tinggi dibandingkan kelompok usia muda, serta perempuan pada rentang usia tertentu memiliki utilisasi layanan yang lebih tinggi dibandingkan laki-laki. Secara kuantitatif, koefisien empiris pada perempuan usia 21–60 tahun sebesar 3,10 lebih tinggi dibandingkan koefisien dalam Peraturan Menteri Kesehatan Nomor 3 Tahun 2023 sebesar 1,18 dengan selisih 1,92. Selain itu, peserta dengan diagnosis penyakit kronis seperti diabetes melitus dan hipertensi menunjukkan tingkat kunjungan yang lebih tinggi dibandingkan peserta tanpa diagnosis tersebut.
Kesimpulan: Kesimpulan penelitian ini menunjukkan bahwa selain faktor demografi, diagnosis penyakit juga berpengaruh signifikan terhadap kebutuhan layanan kesehatan di FKTP. Oleh karena itu, diperlukan pengembangan sistem penyesuaian risiko dalam penetapan tarif kapitasi yang mempertimbangkan faktor diagnosis untuk meningkatkan keadilan dan efisiensi pembiayaan dalam program JKN.
Background: The capitation payment system at Primary Healthcare Facilities (Fasilitas Kesehatan Tingkat Pertama / FKTP) within Indonesia’s National Health Insurance (Jaminan Kesehatan Nasional / JKN) program is currently determined based on demographic factors, namely age and sex, as regulated in Minister of Health Regulation No. 3 of 2023. However, variations in disease diagnoses may influence healthcare service needs and have not been fully incorporated into capitation coefficient adjustments.
Objective: This study aimed to analyze the suitability of capitation coefficients in Minister of Health Regulation No. 3 of 2023 with patterns of healthcare utilization at FKTP based on demographic factors (age and sex) as well as diagnoses of diabetes mellitus and hypertension.
Methods: This study employed a quantitative cross-sectional design using secondary data from BPJS Kesehatan for 2023–2024. The study sample consisted of 255 FKTP out of a total of 341 FKTP in Semarang City. Data were analyzed using negative binomial regression on total FKTP visits by incorporating demographic factors, diagnosis, type of FKTP, and year of service. Empirical capitation coefficients were then calculated from contact rates and compared with the coefficients stipulated in Minister of Health Regulation No. 3 of 2023.
Results: The findings showed that older age groups had higher healthcare utilization than younger age groups, and females in certain age groups had higher utilization than males. Quantitatively, the empirical coefficient for females aged 21–60 years was 3.10, higher than the coefficient in Minister of Health Regulation No. 3 of 2023 (1.18), with a difference of 1.92. In addition, participants with chronic diseases, particularly diabetes mellitus and hypertension, showed higher healthcare utilization than those without such diagnoses.
Conclusion: In addition to demographic factors, disease diagnoses also significantly influenced healthcare service needs at FKTP. Therefore, the development of a risk adjustment system in capitation payment setting that incorporates diagnostic factors is needed to improve fairness and financing efficiency in the JKN program.
