Anemia is one of the most prevalent hematological disorders worldwide and continues to represent a significant public health concern, particularly among adolescent females, pregnant women, and children. In recent years, Machine Learning techniques have increasingly been utilized to support the classification of anemia types. However, the classification process is challenged by the issue of class imbalance within diagnostic datasets. This imbalance causes classification models to exhibit a bias toward majority classes, resulting in suboptimal Recall performance across minority classes. Previous studies reported this limitation, achieving approximately 74?curacy and 59% Recall despite the implementation of the Synthetic Minority Over-sampling Technique (SMOTE). Nevertheless, SMOTE remains less effective in multiclass scenarios because it does not sufficiently account for inter-class variation and class boundary distributions. Furthermore, Adaptive Synthetic Sampling (ADASYN), as an adaptive oversampling technique, is primarily optimized for binary classification and has not been able to comprehensively address the complexity of inter-class relationships in multiclass imbalanced datasets.

To overcome these limitations, this study proposes a modified ADASYN oversampling approach (Modified), which extends the standard ADASYN algorithm to handle multiclass imbalanced data using a pairwise strategy. The proposed method was implemented across six classification algorithms, namely Random Forest, Support Vector Machine, Logistic Regression, Decision Tree, K-Nearest Neighbors, and Naïve Bayes. Model performance was evaluated using classification reports to analyze changes in Precision, Recall, and F1-score before and after the application of the oversampling technique.

The experimental results demonstrate that the Modified method significantly improves overall classification performance, particularly for models that are sensitive to changes in data distribution, such as Random Forest, Support Vector Machine, and Decision Tree. In the Random Forest model, the proposed approach improved Accuracy from 69.09% to 80% and increased Recall from 50.28% to 68.11%. Decision Tree also exhibited substantial improvement, with an increase of more than 11 percentage points in Recall, while Support Vector Machine showed moderate enhancement across both evaluation metrics. These findings indicate that the Modified approach effectively addresses the issue of low Accuracy and Recall identified in previous studies, thereby demonstrating greater suitability for handling multiclass imbalance problems in anemia classification datasets.
