HUBUNGAN KADAR LAKTAT TERHADAP ANGKA KEJADIAN GAGAL GINJAL AKUT PASCA BEDAH JANTUNG DENGAN MESIN PINTAS JANTUNG PARU DI RSUP DR. SARDJITO YOGYAKARTA

Latar Belakang: Bedah jantung dengan mesin pintas jantung-paru (cardiopulmonary
bypass/PJP) merupakan prosedur kompleks yang dapat memicu respons inflamasi sistemik,
gangguan perfusi jaringan, dan akumulasi laktat akibat metabolisme anaerob. Kondisi
hipoperfusi sistemik yang tercermin dari peningkatan kadar laktat pascaoperasi telah
diidentifikasi sebagai salah satu faktor yang berhubungan dengan kejadian komplikasi
organ, termasuk gagal ginjal akut. Cardiac surgery-associated acute kidney injury (CSA-
AKI) merupakan komplikasi serius pasca bedah jantung yang meningkatkan morbiditas,
mortalitas, dan durasi perawatan di unit perawatan intensif.
Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan kadar laktat dengan angka
kejadian CSA-AKI serta menentukan nilai cut-off kadar laktat sebagai penanda risiko
terjadinya CSA-AKI pada pasien pasca bedah jantung dengan mesin pintas jantung-paru di
RSUP Dr. Sardjito Yogyakarta.
Metode: Studi kohort prospektif (Desember 2025–Februari 2026) pada pasien bedah
jantung elektif di RSUP Dr. Sardjito. Kriteria inklusi: usia ?18 tahun, NYHA
dan eGFR ?60 mL/menit/1,73 m². CSA-AKI didefinisikan berdasarkan kriteria KDIGO
2012. Kadar laktat diukur secara serial pada 2 jam, 6 jam, dan 24 jam pasca PJP. Analisis
menggunakan uji statistik komparatif dan kurva Receiver Operating Characteristic(ROC).
Hasil: Subjek penelitian berjumlah 43 subjek (dari total 45 pasien, 2 dropout). Rerata usia
46,84 ± 12,40 tahun, laki-laki (60,5%), EF 61,97 ± 9,17%. Prosedur didominasi CABG
(37,2%) dan katup (37,2%), durasi PJP 96,13 ± 23,71 menit. Angka CSA-AKI sebesar
65,1%. Analisis ROC laktat 6 jam pasca PJP menunjukkan AUC 0,800 (p=0,001) dengan
cut-off optimal 3,44 mmol/L (sensitivitas 71,4%, spesifisitas 73,3%), sedangkan kadar
laktat pada waktu lain tidak menunjukkan perbedaan bermakna. Secara bivariat, kelompok
laktat ?3,44 mmol/L memiliki kejadian CSA-AKI signifikan lebih tinggi (83,3% vs 42,1%;
p=0,005; OR 6,88).
Simpulan: Terdapat hubungan positif antara kadar laktat 6 jam pasca PJP dengan angka
kejadian CSA-AKI dan nilai cut-off kadar laktat 6 jam pasca PJP ?3,44 mmol/L berpotensi
sebagai penanda risiko terjadinya CSA-AKI pada pasien pasca bedah jantung dengan
mesin pintas jantung-paru di RSUP Dr. Sardjito Yogyakarta.

Background: Cardiac surgery using cardiopulmonary bypass (CPB) machine is a complex
procedure that can trigger systemic inflammatory responses, tissue perfusion disorders, and
lactate accumulation. Cardiac surgery-associated acute kidney injury (CSA-AKI) is a
serious postoperative complication of cardiac surgery that increases morbidity, mortality,
and duration of intensive care unit (ICU) stay.
Objective: This study aims to determine the relationship between lactate concentration and
the incidence of CSA-AKI and to establish the lactate cut-off value as a risk marker for
CSA-AKI in patients following cardiac surgery using a cardiopulmonary bypass machine
at Dr. Sardjito General Hospital Yogyakarta.
Methods: A prospective cohort study (December 2025--February 2026) on elective
cardiac surgery patients at Dr. Sardjito General Hospital. Inclusion criteria: age ?18 years,
NYHA
KDIGO 2012 criteria. Lactate concentration were measured serially at 2 hours, 6 hours,
and 24 hours post-CPB. Analysis used comparative statistical tests and Receiver Operating
Characteristic (ROC) curve analysis.
Results: Study subjects comprised 43 subjects (from a total of 45 patients, 2 dropouts).
Mean age 46.84 ± 12.40 years, male (60.5%), EF 61.97 ± 9.17%. Procedures were
dominated by CABG (37.2%) and valve surgery (37.2%), with CPB duration 96.13 ±
23.71 minutes. The incidence of CSA-AKI was 65.1%. ROC curve analysis of lactate 6
hours post-CPB showed AUC 0.800 (p=0.001) with optimal cut-off 3.44 mmol/L
(sensitivity 71.4%, specificity 73.3%), while lactate concentration at other time points
showed no significant differences. Bivariate analysis showed that the group with lactate
?3.44 mmol/L had significantly higher CSA-AKI incidence (83.3% vs 42.1%; p=0.005;
OR 6.88).
Conclusion: There is a positive relationship between lactate concentration 6 hours post-
CPB and the incidence of CSA-AKI, and lactate cut-off value 6 hours post-CPB ?3.44
mmol/L has potential as a risk marker for CSA-AKI in patients following cardiac surgery
with a cardiopulmonary bypass machine at Dr. Sardjito General Hospital Yogyakarta.
