Hubungan Antara Kejadian Drug Related Problems Dengan Outcome Klinis Pada Pasien Rawat Inap Dengan Skizofrenia Di RSJ Prof. Dr. Soerojo Magelang

Latar Belakang: Prevalensi skizofrenia terus meningkat. Penggunaan obat pada pasien skizofrenia dalam waktu yang lama dapat menyebabkan masalah terkait obat atau Drug Related Problems (DRPs). DRPs merupakan keadaan yang melibatkan terapi obat yang berpotensi mengganggu outcome klinis. Outcome klinis yang perlu dipantau dari pasien skizofrenia adalah skor The Positive and Negative Syndrome Scale-Excited Component (PANSS-EC). 

Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara kejadian DRPs dengan outcome klinis (PANSS-EC) pada pasien skizofrenia rawat inap.

Metode: Jenis penelitian ini adalah analitik dengan rancangan retrospective cohort. Pengambilan data melalui catatan rekam medis periode Januari 2021 – Mei 2025. Pengambilan sampel menggunakan metode purposive sampling, kemudian sampel dibagi menjadi kelompok DRP dan non DRP. DRP yang diamati adalah potensial dan aktual sesuai dengan PCNE V9.00. Outcome klinis dinilai berdasarkan penurunan skor PANSS-EC di rekam medis. Dilakukan uji chi square untuk mengetahui hubungan antara kejadian DRP dengan outcome klinis pasien.

Hasil dan Pembahasan: Didapatkan 110 pasien termasuk kriteria inklusi, selanjutnya dibagi menjadi kelompok DRP 53 pasien dan non DRP 57 pasien. Kategori DRP yang terjadi pada penelitian ini adalah keamanan pengobatan (86,4%), pemilihan obat (5,1%), dan penentuan dosis (8,5%). Sebagian besar pasien memiliki outcome klinis membaik (56,4%). Terdapat hubungan yang signifikan antara kejadian DRP dengan outcome klinis (p = 0,001). Kelompok DRP 2,1 kali berisiko tidak membaik dibandingkan dengan kelompok yang tidak mengalami DRP (RR 2,1; 95% CI 1,345-3,439). Kesimpulan: Terdapat hubungan antara kejadian DRP dan outcome klinis pada pasien skizofrenia. Pasien yang mengalami DRP berisiko mengalami outcome klinis tidak membaik dibandingkan pasien yang tidak mengalami DRP.

Background: The prevalence of schizophrenia continues to rise. The use of drugs in schizophrenia patients for a long time can cause drug-related problems (DRPs). DRPs are conditions that involve drug therapy that have the potential to interfere with clinical outcomes. The clinical outcome that needs to be monitored from schizophrenia patients is the Positive and Negative Syndrome Scale-Excited Component (PANSS-EC) score. 

Objective: This study aimed to determine the relationship between the incidence of DRPs and clinical outcomes (PANSS-EC) in hospitalized schizophrenia patients.

Methods: This type of research was analytical with a retrospective cohort design. Data collection through medical records for the period January 2021 – May 2025. Sampling was done using the purposive sampling method, and then the samples were divided into DRP and non-DRP groups. The observed DRP is potential and actual in accordance with PCNE V9.00. Clinical outcomes were assessed based on a decrease in PANSS-EC scores in medical records. A chi-square test was performed to determine the relationship between the incidence of DRP and the patient's clinical outcomes.

Results and Discussion: 110 patients were obtained, including those who met the inclusion criteria, and then divided into a group of 53 DRP patients and 57 non-DRP patients. The DRP categories that occurred in this study were treatment safety (86.4%), drug selection (5.1%), and dosage determination (8.5%). Most patients had improved clinical outcomes (56.4%). There was a significant association between the incidence of DRP and clinical outcomes (p = 0.001). The DRP group was 2.1 times less likely to improve compared to the non-DRP group (RR 2.1; 95% CI 1.345-3.439). Conclusions: There is a relationship between the incidence of DRP and clinical outcomes in schizophrenic patients. Patients who experience DRP are at risk of  not improving clinical outcomes compared to patients who do not experience DRP.
