Expression of Elastin on Dartos Tissue Elasticity in Buried Penis Pediatric Patients

Penis terpendam digambarkan sebagai kondisi di mana batang penis terperangkap dalam jaringan lemak pubis yang menyebabkan penis tampak kecil. Masalah ini akan memengaruhi aspek psikologis pasien dalam memandang citra tubuh mereka. Tujuan utama terapi adalah untuk mencapai fungsi penis normal dan memperbaiki kosmetik penis. Beberapa penelitian telah melaporkan keterlibatan kelainan jaringan dartos dalam kejadian hipospadia dan penis terpendam. Kurangnya elastisitas jaringan dartos menyebabkan penis tertarik ke dalam jaringan sekitarnya, sehingga memberikan penampilan penis terpendam. Sebagai komponen utama struktur fibrosa korpus kavernosum penis, menarik untuk mengetahui apakah elastin berperan dalam patogenesis penis terpendam.

Tujuan:

Tujuan penelitian ini adalah untuk menentukan ekspresi elastin dan pengaruh ekspresi elastin pada pasien anak dengan penis terpendam.

Bahan dan metode

Metode potong lintang dilakukan dengan populasi penelitian pasien anak di Rumah Sakit Umum Dr. Sardjito, JIH, dan Rumah Sakit Siloam. Sampel diperoleh selama prosedur untuk kasus penis terpendam dan sunat untuk penis normal. Ekspresi elastin ditentukan dengan qPCR. Data dianalisis untuk distribusi normalnya menggunakan uji Shapiro–Wilk. Uji Mann–Whitney U digunakan untuk data yang tidak terdistribusi normal, p < 0>

Hasil:

Pada kelompok penis terpendam (BP), usia median pasien adalah 8,75 tahun (rentang: 1,0–12,8 tahun), yang lebih tinggi daripada kelompok kontrol, dengan usia median 4,69 tahun (rentang: 1,12–12,09 tahun). Ekspresi elastin pada jaringan dartos menunjukkan tingkat ekspresi yang lebih rendah pada kelompok BP, dengan nilai median 0,00263 (rentang: 0,0000095–0,0733), dibandingkan dengan kelompok kontrol, yang menunjukkan nilai median 6,76 (rentang: 2,46–27,86); perbedaan ini signifikan secara statistik (p < 0>

Kesimpulan

Berdasarkan analisis deskriptif yang dilakukan dalam penelitian ini, dapat disimpulkan bahwa ekspresi elastin pada kelompok ini menunjukkan distribusi non-normal. Jika dibandingkan secara deskriptif dengan kelompok kontrol, ekspresi elastin pada jaringan dartos pasien penis terpendam jauh lebih rendah daripada yang diamati pada pasien anak dengan penis normal.

Buried penis is described as a condition where the shaft of the penis is trapped in the pubic fatty tissue which causes the penis to appear small. This problem will affect the patient's psychological aspects in viewing their body image. The main goal of therapy is to achieve normal penile function and improve penile cosmetics. Several studies have reported the involvement of dartos tissue abnormalities in the incidence of hypospadias and buried penis. The lack of elasticity of the dartos tissue causes the penis to be pulled into the surrounding tissue, giving the appearance of a buried penis. As the main component of the fibrous structure of the corpus cavernosa of the penis, it is interesting to know whether elastin plays a role in the pathogenesis of buried penis.
Objective : 

The aim of this study was to determine the expression of elastin and the influence of elastin expression in pediatric patients with buried penis. 

Material and methods : 

A cross-sectional method was conducted with the study population for pediatric patients at Dr. Sardjito General Hospital, JIH, and Siloam Hospital. The sample was obtained during a procedure for a case of buried penis and circumcision for normal penis. The expression of elastin was determined by qPCR. Data were analyzed for their normality distribution using the Shapiro–Wilk test. Mann–Whitney U test were used for the data that were not normally distributed, p < 0>

Result : 

In the buried penis (BP) group, the median age of patients was 8.75 years (range: 1.0–12.8 years), which was higher than that of the control group, with a median age of 4.69 years (range: 1.12–12.09 years). Elastin expression in dartos tissue exhibited lower expression levels in the BP group, with a median value of 0.00263 (range: 0.0000095–0.0733), compared to the control group, which showed a median value of 6.76 (range: 2.46–27.86); this difference was statistically significant (p <0>

Conclusion 

Based on the descriptive analysis conducted in this study, it can be concluded that Elastin expression in this group demonstrated a non-normal distribution. When descriptively compared with the control group, elastin expression in dartos tissue of buried penis patients was markedly lower than that observed in pediatric patients with normal penis.
