Potensi Antioksidan Ekstrak Etanol Buah Duwet (Syzygium Cumini L.) pada Tikus Model Glaukoma

Glaukoma adalah gangguan progresif heterogen yang ditandai dengan apoptosis retinal ganglion cell (RGC) dan neuropati optik spesifik yang terkait dengan penyempitan diskus optikus hingga berakibat hilangnya fungsi penglihatan. Buah duwet memiliki kadar antosianin yang tinggi, berperan dalam aktivitas antioksidan. Penelitian ini bertujuan untuk menganalisis efektivitas potensi ekstrak etanol buah duwet terhadap terapi glaukoma melalui metode in vivo. Penelitian eksperimental in vivo menggunakan 30 ekor tikus. Tikus dibagi menjadi 6 kelompok perlakuan yakni 1) kelompok kontrol sehat, 2) kontrol sakit, 3) kontrol terapi (acetazolamide 4,5 mg/200gBB), 4) ekstrak buah duwet 125 mg/kgBB, 5) ekstrak buah duwet 250 mg/kgBB, 6) ekstrak buah duwet 500 mg/kgBB peroral. Induksi hewan glaukoma dengan injeksi deksametason (30 µL) via subkonjungtiva sebanyak dua kali pengulangan setiap minggunya. Perlakuan dilakukan selama 28 hari dengan pengamatan tanda klinis setiap hari dan tekanan intraokular (TIO) setiap minggu. Setelah 28 hari perlakuan, sampel darah diambil untuk pengujian kadar reactive oxygen spesies (ROS), malondialdehid (MDA), dan superoksida dismutase (SOD). Setelah tikus dieutanasi, mata dienukleasi dan dibuat preparat histologis untuk pewarnaan hematoksilin-eosin (HE) dan imunohistokimia (IHC) dengan antibodi anti-nuclear factor erythroid 2-related factor 2 (Nrf2). Data perubahan klinis pada kelompok dianalisis secara deskripsi. Data kuantitatif TIO, MDA, SOD, ROS, ketebalan lapisan retina pada preparat HE secara statistik dengan SPSS menggunakan one way ANOVA, dan data semi-kuantitatif immunoekspresi antibodi anti-Nrf2 pada preparat IHC dianalisa dengan non-parametrik. Hasil penelitian menunjukkan bahwa induksi glaukoma dengan deksametason menyebabkan peningkatan TIO tanpa perubahan klinis. Deksametason juga meningkatkan kadar MDA dan ROS, menurunkan kadar SOD dan penipisan lapisan retina tikus model glaukoma. Terapi dengan ekstrak buah duwet pada tikus model glaukoma dapat menurunan TIO secara signifikan. Selain itu, potensi antioksidan dari ekstrak buah duwet dapat menurunkan kadar MDA, ROS, serta meningkatkan SOD secara signifikan dibandingkan kontrol sakit. Berdasarkan analisis histopatologis retina, ekstrak buah duwet dapat menebalkan lapisan retina tikus model glaukoma yang mengalami penipisan. Meskipun demikian, hasil IHC menunjukkan bahwa immunoekspresi Nrf2 tidak berbeda signifikan baik pada retina tikus model glaukoma tanpa atau dengan terapi ekstrak buah duwet. Berdasarkan hasil tersebut, disimpulkan bahwa ekstrak buah duwet berpotensi sebagai terapi glaukoma melalui penurunan TIO, perbaikan stress oksidatif, dan meningkatkan ketebalan layer retina.
Glaucoma is a heterogeneous progressive disorder characterized by apoptosis of retinal ganglion cells (RGCs) and specific optic neuropathy associated with narrowing of the optic disc resulting in visual loss. Duwet fruit has high anthocyanin content, which plays a role in antioxidant activity. This study aims to determine the potential effectiveness of duwet fruit ethanol extract on glaucoma therapy through in vivo methods. The in vivo experimental study used 30 rats. The rats were divided into 6 treatment groups, namely 1) negative control group, 2) positive control, 3) therapy control (acetazolamide 4.5 mg/200gBW), 4) duwet fruit extract 125 mg/kgBW, 5) duwet fruit extract 250 mg/kgBW, 6) duwet fruit extract 500 mg/kgBW orally. Induction of glaucoma in animals by subconjunctival injection of dexamethasone (30 µL) twice weekly. Treatment was carried out for 28 days by observing clinical signs daily and intraocular pressure (IOP) weekly. After 28 days of treatment, blood samples were taken to measure the levels of reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD). After the mice were euthanized, the eyes were enucleated and histological slides were made for hematoxylin-eosin (HE) staining and immunohistochemistry (IHC) with anti-nuclear factor erythroid 2-related factor 2 (Nrf2) antibodies. Data on clinical changes in the groups were described descriptively. Quantitative data on IOP, MDA, SOD, ROS, retinal layer thickness in HE slides were statistically analyzed using SPSS using one-way ANOVA, and semi-quantitative data on anti-Nrf2 antibody immunoexpression in IHC slides were analyzed non-parametrically. The results showed that glaucoma induction with deksametason caused an increase in IOP without clinical changes. Deksametason also increases MDA and ROS levels, decreases SOD levels, and thins the retinal layer in glaucoma model rats. Therapy with duwet fruit extract in glaucoma model rats can significantly reduce IOP. In addition, the antioxidant potential of duwet fruit extract can significantly reduce MDA and ROS levels and increase SOD compared to diseased controls. Based on retinal histopathological analysis, duwet fruit extract can thicken the retinal layer in glaucoma model rats that experience thinning. However, IHC results showed that Nrf2 immunoexpression was not significantly different in the retina of glaucoma model rats without or with duwet fruit extract therapy. Based on these results, it is concluded that duwet fruit extract has the potential as a glaucoma therapy by reducing IOP, improving oxidative stress, and increasing retinal layer thickness.
