Kadar Glukosa Darah Preoperatif sebagai Prediktor Kesintasan pada Glioma

Latar Belakang: Glioma merupakan tumor otak primer yang paling umum dengan prognosis yang bervariasi. Kadar glukosa darah preoperatif diduga memengaruhi agresivitas tumor dan kesintasan pasien melalui jalur inflamasi dan stres oksidatif.

Tujuan: Penelitian ini bertujuan untuk menganalisis peran kadar glukosa darah rata-rata preoperatif sebagai prediktor kesintasan pada pasien glioma.

Metode: Studi kohort retrospektif dilakukan terhadap 224 pasien glioma yang menjalani reseksi tumor di RSUP Dr. Sardjito Yogyakarta selama periode 2017–2025. Kadar glukosa darah preoperatif dikategorikan menggunakan nilai ambang (cutoff) 113,5 mg/dL. Analisis kesintasan dilakukan menggunakan metode Kaplan-Meier dengan uji log-rank, sementara analisis faktor prognostik multivariat menggunakan model regresi Cox.

Hasil: Median kesintasan pasien glioma berbeda bermakna antara derajat rendah (47,5 bulan) dan derajat tinggi (7,4 bulan; p < 0 xss=removed>60 tahun (HR 4,146; p < 0 xss=removed>

Kesimpulan: Kadar glukosa darah preoperatif merupakan prediktor yang independen terhadap kesintasan pada pasien glioma derajat tinggi.

Background: Glioma is the most common primary brain tumor with varying prognosis. Preoperative blood glucose levels are hypothesized to influence tumor aggressiveness and patient survival through inflammatory pathways and oxidative stress.

Objective: This study aims to analyze the role of mean preoperative blood glucose levels as a predictor of survival in glioma patients.

Methods: A retrospective cohort study was conducted on 224 glioma subjects undergoing tumor resection at RSUP Dr. Sardjito Yogyakarta from 2017 to 2025. Preoperative blood glucose levels were categorized using a cutoff value of 113.5 mg/dL. Survival analysis was performed using the Kaplan-Meier method with the log-rank test, while multivariate prognostic factor analysis was conducted using the Cox regression model.

Results: Overall survival differs significantly between low-grade (47.5 months) and high-grade (7.4 months; p < 0 xss=removed>60 years (HR 4.146; p < 0 xss=removed>

Conclusion: Preoperative blood glucose level is an independent predictor of survival in high grade glioma patients.
