Analisis pengelolaan obat prajurit korban tempur dan latihan tempur di Unit Rawat Inap Kedokteran Militer Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Jakarta

Hasil survai pendahuluan mengindikasikan adanya permasalahan dalam pengelolaan obat di unit rawat inap dokmil RSPAD GS. Oleh karena itu perlu dilakukan penelitian mendalam untuk menemukan permasalahan yang sebenarnya agar bisa ditemukan solusi yang komprehensif dalam hal pengelolaan obatnya. Penelitian ini merupakan penelitian non eksperimental dengan rancangan deskriptif analisis, bersifat eksploratif dengan menggunakan data retrospektif dan prospektif. Data primer diperoleh dari hasil observasi dan wawancara mendalam dengan pihak RSPAD GS, dokter, perawat, petugas depo farmasi maupun dengan pasien langsung. Sedangkan data sekunder diperoleh dari dokumen yang ada di unit kedokteran militer dan IFRS GS berupa lembar resep maupun laporan pemakaian dan permintaan obat-obatan. Hasil penelitian menunjukkan belum ada anggaran khusus obat-obatan dalam perencanaan obat di unit rawat inap dokmil RSPAD GS. Prosentase perbandingan jumlah obat dalam perencanaan dengan kenyataan pakai, 86,27%. Pengadaan obat di unit rawat inap dokmil berasal dari anfrah obat dari IFRS GS dan restitusi obat. Frekuensi pengadaan tiap item obat tertinggi dari Lembar Daftar Permintaan (LDP) obat mencapai 11 kali dan terendah 1 kali. Frekuensi pengadaan tiap item obat tertinggi dari restitusi adalah 7 kali dan terendah 1 kali. Frekuensi kesalahan administrasi pada LDP mencapai 3,34%, sedangkan frekuensi kesalahan administrasi pada proses restitusi mencapai 2,50%. Distribusi obat di unit rawat inap dokmil RSPAD GS menggunakan sistem UDD. Waktu yang dibutuhkan untuk melayani resep sampai ke pasien 28,7 menit. Prosentase obat yang bisa diserahkan kepada pasien 98,69%. Prosentase obat yang dilabeli dengan benar 98,62%. Prosentase keluhan pasien terhadap pelayanan farmasi mencapai 3,33%. Jumlah rata-rata item obat per lembar resep adalah 3,13. Prosentase penulisan resep dengan obat generik 52,30%. Prosentase penulisan resep dengan obat antibiotika 38,21%. Prosentase penulisan resep dengan obat injeksi 18,33 %. Prosentase penulisan resep dengan obat vitamin 12,91%. Prosentase penulisan resep dengan obat sesuai formularium RSPAD GS 72,42%. Prosentase pengetahuan pasien tentang manfaat obat 63,33%.
In-patient unit of military medical is in-patient unit in Gatot Soebroto Army Center Hospital (RSPAD GS) which particularly nurses the war and war practise victim soldiers. The result of introduction survey indicates many problems of the management of medicines in unit of military of RSPAD GS. Therefore, it is necessary to do profound study to find out the real problems so that it can be found the comprehensive solutions in the handling of its medicine. This research is a explorative non experimental one using the plan of descriptive analysis, using prospective and retrospective data. The primary data was obtained from the result of observation and interview deeply with the management of RSPAD GS, doctors, nurses, employees of pharmacy department and with the patients directly. Whereas the secondary data was obtained from the existing documents in military medical unit and pharmacy department of RSPAD GS in the forms of presciption sheet, as well as report of usage and inquiry of medicine. The result of the research showed the absence of special budget of medicine in the medicine plan in this unit. The percentage comparison between the amount of medicine in planning and actual use was 86,27%. The availability of medicine in this unit came from the medicine of pharmacy department (IFRS GS) and medicine restitution. The frequency of availability of each highest medicine item from sheet of list request (LDP) of medicine reached 11 times and the lowest once. The frequency of availability of the highest medicine item from restitution was 7 times and the lowest was once. The frequency of misadministration on the LDP reached 3,34%. The frequency of misadministration on the restitution reached 2,50%. The distribution of medicine in this unit used unit dose dispensing (UDD) system. The time needed to serve prescriptions to patiens was 28,7 minutes. The percentage of medicine which could be given to patients is 98,69%. The percentage of the correct labeled medicine was 98,62%. The percentage of patien’s complain to the pharmacy service was 3,33%. The average of each medicine per prescription was 3,13. The percentage of prescribing generic medicine was 52,30%. The percentage of prescribing antibiotic medicine was 38,21%. The percentage of prescribing injection medicine was 18,33%. The percentage of prescribing vitamin medicine was 12,91%. The percentage of prescribing medicine in line with RSPAD GS formulary was 72,42%. The percentage of patient knowledge about the benefit of medicine was 63,33%.
