ERINGKATAN RISIKO OBAT BETA-2 AGONIST TERDAFTAR BPOM YANG MENGANDUNG SUBSTANSI TERLARANG MENURUT WORLD ANTI DOPING CODE INTERNATIONAL STANDARD PROHIBITED LIST BAGI ATLET

Latar Belakang : Doping merupakan produk atau zat obat yang dapat meningkatkan performa seorang atlet dan merupakan zat yang dilarang dalam kompetisi olahraga resmi di dunia. Beta-2 Agonist merupakan jenis obat yang sering digunakan untuk memperlebar saluran pernapasan dan merupakan obat yang termasuk dalam kategori doping. Di Indonesia sendiri, 1,6?ri total populasi (atau 12 juta penduduk di Indonesia) merupakan penderita asma. Sehingga penggunaan Beta-2 Agonist rentan dianggap sebagai penyalahgunaan doping.

Metode : Penelitian ini menggunakan metode analisis deskriptif yang diolah dan dibuat dalam bentuk ranking Naranjo Adverse Drug Reaction (ADR) Probability Scale dan Jackson 5 (J5) System yang dimodifikasi. Populasi penelitian ini adalah produk obat yang terdaftar di Badan Pengawasan Obat dan Makanan (BPOM) pada periode Oktober 2023 hingga Januari 2024, dengan kriteria inklusi berupa produk yang didaftarkan sebagai obat di BPOM dan kategori Beta-2 Agonist.

Hasil : Fenoterol, Formoterol, Indacaterol, Isoxsuprine, Olodaterol, Procaterol, Salbutamol, Salmeterol, Terbutaline, Tretoquinol, dan Vilanterol adalah produk kelas Beta-2 Agonist terlarang WADA yang beredar secara resmi di Indonesia. Dalam pemeringkatan, ditemukan obat dengan risiko menengah-tinggi dan aksesibilitas menengah-rendah (Procaterol Oral, Isoxsuprine Oral, Fenoterol Inhalasi, Isoxsuprine Intravena, Procaterol Inhalasi, dan Tretoquinol Oral), risiko menengah-rendah dan aksesibilitas menengah-rendah (Terbutaline Oral, Indacaterol Inhalasi, Olodaterol Inhalasi, Terbutaline Inhalasi, dan Terbutaline Intravena), dan risiko menengah-rendah dan aksesibilitas rendah (Salbutamol Oral, Salbutamol Inhalasi, Salmeterol Inhalasi, Formoterol Inhalasi, Salbutamol Intravena, dan Vilanterol).

Kesimpulan : Fenoterol, Formoterol, Indacaterol, Isoxsuprine, Olodaterol, Procaterol, Salbutamol, Salmeterol, Terbutaline, Tretoquinol, dan Vilanterol merupakan substansi doping kelas Beta-2 Agonist yang terdaftar resmi di Indonesia. Terdapat 191 obat terdaftar BPOM yang mengandung substansi doping Beta-2 Agonist. Pemeringkatan risiko obat kelas Beta-2 Agonist yang secara resmi beredar di Indonesia, seluruhnya risiko menengah-tinggi hingga menengah-rendah dengan aksesibilitas menengah rendah hingga rendah.

Background : Doping refers to products or medicinal substances that can enhance an athlete's performance and are prohibited in official global sports competitions. Beta-2 Agonists are a class of drugs frequently used to dilate the airways and are categorized as doping substances. In Indonesia, 1.6% of the total population (approximately 12 million people) suffer from asthma. Consequently, the use of Beta-2 Agonists is highly susceptible to being identified as doping misuse.

Method : This study employs a descriptive analysis method, processed and presented using the modified Naranjo Adverse Drug Reaction (ADR) Probability Scale and the Jackson 5 (J5) System rankings. The study population consists of medicinal products registered with the Indonesian Food and Drug Authority (BPOM) between October 2023 and January 2024. The inclusion criteria were products registered as medicines at BPOM under the Beta-2 Agonist category.

Results : Fenoterol, Formoterol, Indacaterol, Isoxsuprine, Olodaterol, Procaterol, Salbutamol, Salmeterol, Terbutaline, Tretoquinol, and Vilanterol are WADA-prohibited Beta-2 Agonist class products officially circulating in Indonesia. The ranking identified drugs with medium-high risk and medium-low accessibility (Oral Procaterol, Oral Isoxsuprine, Inhaled Fenoterol, Intravenous Isoxsuprine, Inhaled Procaterol, and Oral Tretoquinol); medium-low risk and medium-low accessibility (Oral Terbutaline, Inhaled Indacaterol, Inhaled Olodaterol, Inhaled Terbutaline, and Intravenous Terbutaline); and medium-low risk with low accessibility (Oral Salbutamol, Inhaled Salbutamol, Inhaled Salmeterol, Inhaled Formoterol, Intravenous Salbutamol, and Vilanterol).

Conclusion : Fenoterol, Formoterol, Indacaterol, Isoxsuprine, Olodaterol, Procaterol, Salbutamol, Salmeterol, Terbutaline, Tretoquinol, and Vilanterol are Beta-2 Agonist doping substances officially registered in Indonesia. There are 191 BPOM-registered drugs containing Beta-2 Agonist doping substances. The risk ranking for Beta-2 Agonist drugs officially circulating in Indonesia ranges from medium-high to medium-low risk, with accessibility ranging from medium-low to low.
