Hubungan Nilai Neutrophil-to-Lymphocyte Ratio (NLR) dengan Lama Rawat Inap Pasien Ulkus Kaki Diabetes di RSUP Dr. Sardjito

Latar Belakang: Penyembuhan luka yang kurang baik pada ulkus kaki diabetes dapat memperpanjang masa rawat inap. Prediksi masa rawat inap dapat mendukung peningkatan kualitas layanan kesehatan dan mencegah komplikasi. Neutrophil-to-lymphocyte ratio (NLR) merupakan penanda stres neuroendokrin dan inflamasi yang telah dilaporkan berhubungan secara independen dengan luaran buruk pada ulkus kaki diabetes dan penyakit lain. Penelitian yang meneliti keterkaitan NLR secara langsung dengan lama rawat inap masih terbatas.

Tujuan: Menganalisis hubungan antara nilai NLR dengan lama rawat inap pada pasien dengan ulkus kaki diabetes di RSUP Dr. Sardjito.

Metode: Analisis kohort retrospektif pada populasi pasien ulkus kaki diabetes yang dirawat inap di RSUP Dr. Sardjito pada Januari 2018–Desember 2023. Hubungan variabel independen dan dependen dianalisis dengan uji Chi-square dan regresi logistik. Besar sampel diperoleh dengan total sampling.

Hasil: Sebanyak 577 sampel dilibatkan dalam penelitian ini dengan mayoritas berjenis kelamin laki-laki (54,7%) dan median usia 58 (52–65) tahun. Analisis multivariat dengan regresi logistik menunjukkan bahwa NLR ? 3,565 (OR = 3,837; p < 0 xss=removed xss=removed xss=removed xss=removed>

Kesimpulan: Neutrophil-to-lymphocyte ratio (NLR) ? 3,565 berhubungan secara signifikan dan independen sebagai prediktor lama rawat inap ? 7 hari pada pasien diabetes mellitus tipe 2 dengan ulkus kaki diabetes.

Background: Impaired wound healing in diabetic foot ulcers can prolong the length of hospital stay (LOS). Predicting LOS helps improve the quality of healthcare services and prevents the adverse consequences of prolonged hospitalization. Neutrophil-to-lymphocyte ratio (NLR) is a biomarker that reflects the intensity of neuroendocrine stress and inflammatory response and has been independently associated with poor outcomes in diabetic foot ulcers and other diseases. However, evidence on the direct relationship between patients’ NLR and LOS remains limited.
Objective: To investigate the relationship between the NLR and length of hospital stay of patients with diabetic foot ulcers in Dr. Sardjito General Hospital.
Methods: A retrospective cohort in patients with diabetic foot ulcers (DFU) being hospitalized in Dr. Sardjito General Hospital between January 2018 and December 2023. The relationships between the independent and dependent variables were analyzed using the Chi-square test and logistic regression. The sample size was obtained through total sampling.
Results: A total of 577 samples were included with the majority being male (54.7%) and a median age of 58 (52–65) years. Multivariate analysis using logistic regression showed that NLR ? 3.565 (OR = 3.837; p < 0 xss=removed xss=removed xss=removed xss=removed>
Conclusion: Neutrophil-to-lymphocyte ratio (NLR) ? 3.565 was significantly and independently associated as a predictor of length of stay ? 7 days in type 2 diabetes mellitus patients with diabetic foot ulcers.
