HUBUNGAN SKOR LUND-MACKAY PADA CT SCAN SINUS PARANASAL TERHADAP KUALITAS HIDUP PADA PENDERITA RINOSINUSITIS KRONIS DI RSUP DR. SARDJITO

Latar Belakang: Inflamasi kronis saluran napas atas yang dikenal sebagai rinosinusitis kronis (RSK) sangat memengaruhi kualitas hidup pasien. Sistem skoring Lund-Mackay dapat digunakan untuk mengevaluasi tingkat keparahan penyakit secara anatomis dengan melakukan scan computed tomography (CT) sinus paranasal. Namun, penelitian menyeluruh belum dilakukan tentang hubungan antara temuan radiologis dan persepsi subjektif pasien tentang kualitas hidup. Ini terutama berlaku untuk populasi Indonesia.
Tujuan: Menentukan hubungan skor Lund-Mackay pada CT-scan sinus paranasal dan kualitas hidup pasien RSK berdasarkan skor SNOT-22 di RSUP Dr. Sardjito, Yogyakarta.
Metode: Penelitian observasional analitik dengan pendekatan potong lintang dilakukan terhadap pasien RSK dewasa yang belum menjalani operasi, yang datang ke Poliklinik THT-KL RSUP Dr. Sardjito selama Januari hingga Oktober 2024. Skor Lund-Mackay diperoleh dari hasil CT-scan sinus paranasal, sementara kualitas hidup pasien dinilai dengan kuesioner SNOT-22 versi Bahasa Indonesia. Uji korelasi Pearson dan analisis regresi linier digunakan untuk menganalisis hubungan antar variabel.
Hasil: Dari total 59 pasien yang memenuhi kriteria inklusi dan eksklusi, ditemukan bahwa skor Lund-Mackay berkorelasi positif signifikan dengan skor total SNOT-22 (p < 0>
Kesimpulan: Terdapat hubungan bermakna antara skor Lund-Mackay pada CT-scan sinus paranasal dan penurunan kualitas hidup berdasarkan skor SNOT-22 pada penderita RSK. Hasil ini menunjukkan bahwa skor Lund-Mackay tidak hanya mencerminkan derajat kelainan anatomis, tetapi juga dapat menjadi indikator prognostik terhadap derajat disabilitas subjektif pasien.
Background: Chronic inflammation of the upper respiratory tract known as chronic rhinosinusitis (CRS) significantly affects the quality of life of patients. The Lund-Mackay scoring system can be used to evaluate the anatomical severity of the disease by performing a computed tomography (CT) scan of the paranasal sinuses. However, comprehensive research has not yet been conducted on the relationship between radiological findings and patients' subjective perception of quality of life. This is especially true for the Indonesian population.
Objective: To determine the relationship between Lund-Mackay scores on paranasal sinus CT scans and the quality of life of RSK patients based on SNOT-22 scores at RSUP Dr. Sardjito, Yogyakarta.
Method: An analytical observational study with a cross-sectional approach was conducted on adult RSK patients who had not undergone surgery, who visited the ENT-Head and Neck Polyclinic at RSUP Dr. Sardjito from January to October 2024. The Lund-Mackay score was obtained from the results of a paranasal sinus CT scan, while the patients' quality of life was assessed using the Indonesian version of the SNOT-22 questionnaire. Pearson correlation test and linear regression analysis were used to analyze the relationship between variables.

Conclusion: There is a significant relationship between the Lund-Mackay score on CT-scan of the paranasal sinuses and the decline in quality of life based on the SNOT-22 score in patients with RSK. These results indicate that the Lund-Mackay score not only reflects the degree of anatomical abnormalities but can also serve as a prognostic indicator of the degree of subjective disability in patients.
