Hubungan antara Ekspresi AEG-1 dan CEA pada Kanker Kolorektal Resectable Non-Metastatic dengan Disease Free Survival

Latar belakang: Kanker Kolorektal menempati peringkat keempat sebagai jenis kanker yang terdiagnosis di Indonesia. Astrocyte elevated gene-1 (AEG-1) adalah onkogen yang berperan penting pada proliferasi kanker. CEA berperan sebagai faktor prognosis kanker kolorektal resectabale non-metastatic dan berpotensi berhubungan dengan AEG-1. Oleh karena itu AEG-1 dan CEA berpotensi dapat digunakan sebagai penentuan prognosis pada kanker kolorektal resectable non- metastatic.

Tujuan: Mengetahui hubungan dan ekspresi AEG-1 dan CEA pada Kanker Kolorektal Resectable Non-Metastatic dengan Disease Free Survival. Metodologi penelitian: Penelitian ini merupakan penelitian non-experimental analytic dengan pendekatan metodologi cohort retrospektif. Ekspresi AEG-1 pada spesimen jaringan tumor diperiksa secara qRT-PCR dengan menggunakan Bioline SeniFast SYBR dan SBS Sangon Biotech Co Ltd, Shanghai, China. Ekspresi CEA pada spesimen darah (serum) diperiksa secara ECLIA technique. Disease free survival adalah periode waktu dimana tidak ditemukan lesi baru setelah operasi dilakukan yang dievaluasi dengan pemeriksaan penunjang pada lokoregional maupun metastasis jauh.

Hasil: Pada penelitian ini berdasarkan uji Kaplan Meier didapatkan bahwa pasien kanker kolorektal resectable non-metastatic yang mempunyai AEG-1 dan CEA tinggi memiliki penurunan lebih tajam pada DFS dibanding yang mempunyai AEG-1 dan ACE rendah. Median DFS pada AEG-1 rendah sebesar 35 bulan dan AEG-tinggi sebesar 10 bulan. Adapun median DFS pada CEA rendah adalah 38 bulandan median CEA tinggi adalah 12 bulan. Uji Log rank menunjukkan adanya hubungan yang signifikan antara AEG-1 dan CEA dengan disease free survival (DFS) kanker kolorektal resectable non-metastatic (p=0,000<0 p=0.003 p=0,014).>

Kesimpulan: Terdapat hubungan yang signifikan antara antara ekspresi Astrocyte Elevated Gene-1 (AEG-1) maupun CEA dengan Disease Free Survival (DFS) dan terdapat hubungan antara AEG-1 dengan CEA, adapun pengaruh yang paling signifikan adalah CEA terhadap DSF pada pasien kanker kolorektal resectable non- metastatic.

Background: Colorectal cancer ranks fourth as a type of cancer diagnosed in Indonesia. Astrocyte elevated gene-1 (AEG-1) is an oncogene that plays an important role in cancer proliferation. CEA acts as a prognosis factor for non- metastatic resectable colorectal cancer and is potentially associated with AEG-1. Therefore, AEG-1 and CEA can potentially be used as prognosis determinants in non-metastatic resectable colorectal cancer.

Objective: To determine the association and expression of AEG-1 and CEA in Resectable Non-Metastatic Colorectal Cancer with Disease Free Survival. Research methodology: This study was a non-experimental analytic study with a retrospective cohort methodology approach. AEG-1 expression in tumor tissue specimens was examined by qRT-PCR using Bioline SeniFast SYBR and SBS Sangon Biotech Co Ltd, Shanghai, China. CEA expression in blood (serum) specimens was examined by ECLIA technique. Disease free survival is the period of time when no new lesions were found after surgery, whichwas evaluated by supporting examinations on locoregional and distant metastases.

Results: In this study, based on the Kaplan Meier test, it was found that non- metastatic resectable colorectal cancer patients who had high AEG-1 and CEA had a sharper decrease in DFS than those who had low AEG-1 and ACE. Median DFS in AEG-1 low was 35 months and AEG-high was 10 months. The median DFS for low CEA is 38 months and the median for high CEA is 12 months. The log rank test showed that there was a significant relationship between AEG-1 and CEA with disease free survival (DFS) of non-metastatic resectable colorectal cancer (p=0.000<0 p=0.003 p=0.014).>
