Geriatric patients, characterized by multiple pathologies and complex medication regimens, are at risk of inappropriate prescribing and drug-related problems. The aimed of this study is to develop a clinical pharmacy intervention tailored to long-term medication issues in geriatric patients and to evaluate its effectiveness.
The study consisted of three phases: Phase 1 involved identifying long-term medication-related problems in geriatric patients; Phase 2 focused on developing the Structured Medication Review and Counseling (SMaRC) model and the Geriatric Medication Review (GMaR) application as tools for medication review; and Phase 3 evaluated the effectiveness of the model. The research design included a mixed-method study in Phase 1, a qualitative study in Phase 2, and a quasi-experimental pretest-posttest design with a control group in Phase 3. The study subjects included geriatric patients, pharmacists, and physicians who met the inclusion criteria. The research is conducted at UNS Hospital from August 2023 to November 2024. The instruments used include the Indonesian version of the Living with Medicine Questionnaire (LMQ), MARS-5, EQ-5D-5L, EQ-VAS, counseling forms, and care plan documentation. Effectiveness is assessed based on adherence outcomes, clinical responses, and quality of life. Interviews with patients and pharmacists were conducted as part of the process evaluation.
Geriatric patients experienced a moderate medication burden, with an average LMQ score of 90.4. Chronic medication-related issues in geriatric patients included lack of effectiveness, practical difficulties, treatment fatigue, and non-adherence. Additionally, inappropriate prescribing, adverse drug effects, and drug interactions remain challenges in geriatric care.
