Analisis hambatan perencanaan di Dinas Kesehatan Kota Palangkaraya dalam pembentukan Program Jaminan Pemeliharaan Kesehatan Masyarakat (JPKM)

Latar Belakang: kebijakan Pemerintah Kota Palangka Raya untuk menyelenggarakan program JPKM pada tahun 2004, sebagaimana terdapat di Pola Dasar Pembangunan Daerah Tahun 2001-2003, masih belum terealisasi oleh Dinas Kesehatan selaku stakeholder kunci yang berperan utama memprakarsai perencanaan program JPKM. Di identifikasi kemungkinan faktor hambatan dalam perencanaan program JPKM disebabkan: (1) rendahnya tingkat pemahaman manajerial perencana terhadap program JPKM, (2) rendahnya motivasi manajerial perencana terhadap perencanaan program JPKM, (3) kurangnya sumber daya organisasi meliputi, sumber daya manusia, anggaran dan infrastruktur dari stakeholder. Tujuan: untuk mengetahui (1) tingkat pemahaman dan tingkat motivasi manajerial perencana terhadap program JPKM, dan (2) seberapa besar tingkat pemahaman dan motivasi manajerial perencana terhadap program JPKM dapat mempengaruhi perencanaan program JPKM di Kota Palangka Raya. Metode: jenis penelitian ini kualitatif eksploratif. Subyek penelitian terdiri dari manajerial perencana di Dinas Kesehatan Kota Palangka Raya dengan kriteria 3 orang manajerial perencana di unit utama dan 3 orang manajerial perencana di unit perbantuan. Alat penelitian yang digunakan berupa wawancara mendalan (indepth interview) dan test dalam bentuk kuesioner. Hasil: 1) manajerial perencana di unit utama memiliki pemahaman dan motivasi yang baik terhadap program JPKM, tetapi hasil perencaan program JPKM di unit utama dinilai tidak baik. (2) manajerial perencana di unit perbantuan dinilai belum memiliki pemahaman dan motivasi yang baik terhadap program JPKM, dan diketahui pula hasil perencanaan program JPKM di unit perbantuan dinilai tidak baik. Kesimpulan: 1) rendahnya realisasi perencanaan program JPKM di unit utama, tidak memiliki hubungan signifikan dengan tingginya pemahaman dan motivasi responden terhadap program JPKM, (2) rendahnya hasil perencanaan program JPKM di unit perbantuan, memiliki hubungan signifikan dengan rendahnya pemahaman dan motivasi responden terhadap program JPKM, (3) tidak ada dasar kuat, ketidaksiapan sumber daya organisasi menjadi faktor penghambat pembentukan program JPKM.

Background: The policy of Palangka Raya government to implement the Managed Care program in the year of 2004, as it is mentioned in the Basic Regulation of Regional Development in the year of 2001 – 2003, still not yet realized by the Health Office as the main stakeholder which functioned in initiating the implementation planning. It was identified that the obstacle factor possibility in the Managed Care program planning was caused by: (1) low-level understanding of managerial planner toward Managed Care program, (2) low motivation of managerial planner toward Managed Care program, (3) lack of organization resources which consist of human resources, budget and infrastructure from stakeholder. Objective: To find out (1) the level of understanding and motivation of managerial planner toward Managed Care program, and (2) how far is the understanding and motivation of managerial planner toward Managed Care program could influence the Managed Care program planning in Palangka Raya city. Method: This was a qualitative explorative research. The subjects consists of managerial planner in the Health Office of Palangka Raya with criteria of 3 managerial planner in the main unit and 3 managerial planner in the branch unit. The instrument being used was in depth interview and test with questioner. Result: 1) managerial planner in the main unit had a good understanding and motivation toward Managed Care program although the result of Managed Care program planning in this unit was considered as not good. (2) Managerial planner in branch unit was considered not having a good understanding and motivation toward the Managed Care program, and it was also known that the result of Managed Care program planning in branch unit was considered as not good. Conclusion: 1) low realization of Managed Care program planning in the main unit, did not have significant relationship with the high level of understanding and motivation of respondents toward the Managed Care program, (2) the low result of Managed Care program planning in branch unit had significant relationship with the low level of understanding and motivation of respondent toward the Managed Care program, (3) the unavailability of powerful fondation and non readiness of organization resources became the obstacle factors in forming the Managed Care program.
