Hubungan Nilai Mean Platelet Volume Dan Neutrofil Dengan Amputasi Pada Kaki Ulkus Diabetik

Tujuan: Penelitian ini bertujuan untuk
menganalisis hubungan antara nilai MPV, trombosit, neutrofil, dan monosit
sebagai biomaker inflamasi dengan kejadian dan jenis amputasi pada pasien
dengan ulkus kaki diabetik di RSUP Dr. Sardjito Yogyakarta.
Metode: Desain penelitian yang digunakan
adalah observasional analitik dengan pendekatan case-control, mengambil data
dari pasien DFU yang menjalani amputasi ataupun tidak, di RSUP Dr. Sardjito
Yogyakarta pada periode Januari 2021 hingga Desember 2022. Data yang dianalisis
meliputi nilai MPV, neutrofil, trombosit dan monosit yang diperoleh dari
pemeriksaan darah rutin.
Hasil: Hasil penelitian menunjukkan tidak adanya
hubungan yang signifikan antara nilai MPV, neutrofil, trombosit dengan kejadian
amputasi pada pasien DFU. Sedangkan nilai monosit menunjukan adanya hubungan
signifikan terhadap kejadian amputasi (OR adjusted 2.56; CI 1.03-6.37; p

Kesimpulan: Nilai monosit dapat dijadikan indikator dalam memprediksi kebutuhan amputasi pada pasien dengan ulkus kaki diabetik. Diperlukan penelitian lebih lanjut untuk memperdalam pemahaman mengenai peran biomarker ini dalam pengelolaan klinis DFU.
Background: Diabetes Mellitus (DM) is a metabolic
disease that is increasing in prevalence and has various serious complications,
one of which is diabetic foot ulcer (DFU), which can lead to lower limb
amputation. One of the factors that may influence the amputation decision is
the biomarker values such as Mean Platelet Volume (MPV), neutrophils,
platelets, and monocytes.
Objective: This study aims to analyze the
relationship between MPV, platelet, neutrophil, and monocyte values as inflammatory
biomakers with the incidence and level of amputation in diabetic foot ulcer
patients at RSUP Dr. Sardjito Yogyakarta.
Methods: The research design used was
analytical observational with a case-control approach, taking data from DFU
patients who underwent amputation or not, at Dr. Sardjito Hospital Yogyakarta
in the period January 2021 to December 2022. The data analyzed included MPV,
neutrophil, platelet and monocyte values obtained from routine blood tests.
Results: The results showed no significant relationship between MPV, neutrophil, platelet values and the incidence of amputation in DFU patients. While the value of monocytes showed a significant relationship with the incidence of amputation (adjusted OR 2.56; CI 1.03-6.37; p : 0.043 (p<0>
Conclusion: Monocyte values can be used as an
indicator in predicting the need for amputation in patients with diabetic foot
ulcers. Further research is needed to deepen the understanding of the role of
this biomarker in the clinical management of DFU..
