Basic immunization is a cost-effective intervention to reduce child morbidity and mortality. However, Indonesia's immunization coverage remains below the 90% WHO target. This study examines the impact of household conditions, maternal health behaviors, and geographic disparities on the immunization status of children aged 12-23 months using IDHS 2012 and 2017 data. A binary logistic regression model was applied to analyze factors influencing immunization card ownership and complete basic immunization. Results indicate that higher maternal education, and better economic status significantly increased the likelihood of immunization. Conversely, larger family sizes and higher birth order were negatively associated with immunization completeness. Immunization card and maternal health behavior was a strong determinant; children whose mothers attended at least four ANC visits significantly higher immunization rates. Geographic disparities were evident, with lower immunization rates in Sumatra compared to Java-Bali. The findings highlight the need for targeted interventions, including family-oriented education, improved access to health services, and digital reminder systems in well-connected areas, while community health cadres play a crucial role in remote regions. Future research should adopt longitudinal approaches to better understand causal relationships and assess intervention effectiveness across diverse household contexts.
