Penanganan serentak restorasi mahkota jaket porselin fusi metal dengan inti pasak tuang pada empat gigi insisivus maksila kondisi traumatik

Laporan kasus ini bertujuan untuk menginformasikan penggunaan inti pasak tuang sebagai faktor retensi yang dapat merubah sudut inklinasi mahkota jaket porselin fusi metal pada empat gigi insisivus maksila dengan kondisi traumatik sehingga dapat mengembalikan bentuk anatomi dan fungsi gigi normal kembali di dalam rongga mulut. Pasien laki-laki 25 tahun, datang ke Klinik Konservasi Gigi FKG UGM, ingin merawatkan ke empat gigi insisivus maksila patah dua tahun yang lalu. Perkusi dan palpasi tidak ada rasa sakit, kondisi traumatik. Gambaran radiograf pada empat gigi insisivus maksila terlihat pengisian hermetis, radiolusen di sekitar apeks gigi. Diagnosis gigi insisivus lateralis kiri maksila, gigi insisivus sentralis kiri maksila, gigi insisivus lateralis kanan maksila fraktur Ellis kelas III non vital pasca perawatan saluran akar. Gigi insisivus sentralis kanan maksila fraktur Ellis kelas II non vital pasca perawatan saluran akar. Empat gigi tersebut kondisi traumatik. Dilakukan perawatan dengan restorasi mahkota jaket porselin fusi metal dengan inti pasak tuang secara serentak pada empat gigi insisivus maksila. Kunjungan pertama dilakukan preparasi tonggak, preparasi saluran pasak dan pencetakan tonggak dilakukan secara serentak. Kunjungan ke dua pengepasan inti pasak, penyemenan inti pasak dan pencetakan inti tuang serta menentukan warna gigi dengan vita lumin shade guide. Kunjungan ke tiga pengepasan dan penyemenen mahkota jaket porselin fusi metal. Setelah satu minggu dilakukan kontrol, empat gigi insisivus maksila tidak ada rasa sakit, tidak ada traumatik oklusi, warna tidak berubah. Gambar radiograf tidak ada garis fraktur pada akar gigi. Evaluasi setelah satu bulan, empat gigi insisivus maksila kondisi traumatik, fungsi gigi dapat dikembalikan, sehingga fungsi mastikasi, fungsi estetik, fungsi bicara dan fungsi perlindungan terhadap jaringan pendukung normal kembali dan pasien merasa puas.

The purpose of this case report was to inform the use of cast post and core as a retention factor, which altered the porcelain fused to metal jacket crown angulation of four fractured upper incisors with traumatic condition, so that they returned to normal anatomically and functionally in the oral cavity. A 25-year old male came to Restorative Dentistry Clinic of Gadjah Mada University looked for treatment of all four upper incisors that were fractured two years before. Percussion and palpation were negative, the upper and lower teeth relation was in traumatic condition. The radiograph showed hermetic obturations with periapical radiolucencies. All four upper incisors were diagnosed as non-vital post endodontic treatment with Ellis Class II fracture of upper right central incisor, whereas the others were Ellis Class III fractures. The treatment was performed concurrently with porcelain fused to metal jacket crown and cast post and core. The first visit was performed with preparation of post space and core construction. Second visit, fitting and cementation of post and core, impression taking and shade selection. Third visit was fitting and cementation of porcelain fused to metal jacket crown. Control was made one week after crown insertion, the results showed no pain for all four upper incisors, no traumatic occlusion, and no shade changes. The radiograph displayed no root fracture. A one month follow-up revealed that all four upper incisors with traumatic condition were back to normal function which were mastication, esthetics, speech and protection of underlying tissues, and the patient was satisfied
