Background: To date, there are two therapy protocols for Wilms tumor, including the Children's Oncology Group (COG) and the International Society of Pediatric Oncology (SIOP). Preoperative chemotherapy in the SIOP protocol has been reported to reduce surgical complications. Further research is necessary to analyze the profile and outcome of Wilms tumor at Dr. Sardjito Hospital based on the COG and SIOP protocols.

Objective: To determine the profile and outcome of Wilms tumor based on the COG and SIOP protocols at Dr. Sardjito Hospital.

Methods: This retrospective cohort design study involved 29 Wilms tumor patients undergoing treatment at Dr. Sardjito Hospital from January 1, 2015, to July 1, 2024.

Results: A total of 13 subjects (44.8%) underwent COG protocol therapy and 16 (55.2%) subjects underwent SIOP protocol therapy. The number of Wilms tumor patients with mass size ?10 cm before surgery was significantly higher in the COG protocol than in the SIOP protocol (92.3% vs 43.8%; p<0>0.05)

Conclusion: Wilms tumor patients with mass size <10> more common in the COG protocol group compared to the SIOP protocol group. Metastasis was more common in the COG protocol group compared to the SIOP protocol group, but complication and survival rate were not significantly different.
