Analisis Biaya Terapi Diabetes Melitus Tipe 2 di RSUD Kota Yogyakarya dan Faktor yang Memengaruhinya

Diabetes merupakan kondisi dimana glukosa darah didalam tubuh mengalami peningkatan yang melebihi batas normal. Diabetes menjadi salah salah satu ancaman global akibat peningkatan prevalensi diabetes terjadi di seluruh dunia. Diabetes menimbulkan beban ekonomi yang besar, sehingga memerlukan analisis biaya untuk memahami besarnya beban keuangan akibat diabetes melitus Tujuan dari penelitian ini adalah untuk mengetahui biaya terapi diabetes melitus tipe 2, faktor-faktor yang memengaruhi biaya terapi, dan perbedaan biaya medis langsung rawat jalan dan rawat inap diabetes melitus tipe 2 di RSUD Kota Yogyakarta dengan tarif INA-CBG. 

Penelitian ini menggunakan desain penelitian analitik yang menggunakan metode cross sectional. Pengambilan data dilakukan secara retrospektif dari catatan medis dan rincian biaya perawatan pasien dengan diagnosis utama diabetes melitus tipe 2 dengan atau tanpa komplikasi yang memenuhi kriteria inklusi dan eksklusi. Pengambilan sampel dilakukan dengan metode consecutive sampling. Perspektif yang digunakan yaitu perspektif penyedia layanan kesehatan. Penelitian dilakukan pada pasien diabetes melitus tipe 2 yang melakukan kunjungan rawat jalan dan rawat inap di RSUD Kota Yogyakarta. Analisis data terdiri dari univariat, bivariat, dan  multivariat.

Hasil penelitian diperoleh jumlah pasien diabetes melitus tipe 2 sebanyak 102 pasien, terdiri dari 60 pasien dengan kunjungan rawat jalan dan 42 pasien dengan kunjungan rawat jalan dan rawat inap. Biaya terapi/pasien/tahun terapi diabetes melitus tipe 2 yaitu Rp10.802.763. Biaya untuk satu kali kunjungan rawat jalan yaitu Rp587.721. Biaya untuk satu kali kunjungan rawat inap yaitu Rp8.206.184. Faktor yang memengaruhi biaya medis langsung terapi diabetes melitus tipe yaitu pada variabel usia, jenis antihiperglikemi, dan frekuensi kunjungan. Terdapat ketidaksesuaian antara biaya riil dengan tarif INA-CBG pada kunjungan rawat jalan dan rawat inap dengan selisih negatif masing-masing sebesar Rp29.611.945 dan Rp78.761.088.

Diabetes is a condition where blood glucose in the body increases beyond normal limits. Diabetes has become one of the global threats due to the increasing prevalence of diabetes worldwide. Diabetes causes a large economic burden, so it requires cost analysis to understand the magnitude of the financial burden due to diabetes mellitus. The purpose of this study was to determine the cost of type 2 diabetes mellitus therapy, factors that affect the cost of therapy, and the difference in direct medical costs of outpatient and inpatient type 2 diabetes mellitus at Yogyakarta City Hospital with INA-CBG rates. 

This study uses an analytic research design that uses the cross sectional method. Data were collected retrospectively from medical records and treatment cost details of patients with a primary diagnosis of type 2 diabetes mellitus with or without complications who met the inclusion and exclusion criteria. Sampling was done by consecutive sampling method. The perspective used was the perspective of health care providers. The study was conducted on patients with type 2 diabetes mellitus who made outpatient and inpatient visits at Yogyakarta City Hospital. Data analysis consisted of univariate, bivariate, and multivariate.

The results showed that there were 102 patients with type 2 diabetes mellitus, consisting of 60 patients with outpatient visits and 42 patients with outpatient visits and hospitalizations. The cost of therapy/patient/year of type 2 diabetes mellitus therapy is IDR 10,802,763. The cost for one outpatient visit is IDR 587,721. The cost for one inpatient visit is IDR 8,206,184. Factors that influence the direct medical costs of type 2 diabetes mellitus therapy are the variables of age, type of antihyperglycemic, and frequency of visits. There is a mismatch between real costs and INA-CBG rates for outpatient and inpatient visits with a negative difference of Rp29,611,945 and Rp78,761,088, respectively.
