Korelasi Persentase Klas Eritrosit Poikilositosis dengan Kadar Saturasi Transferin dan Ferritin

Latar Belakang: Hematology analyzer merupakan alat otomatis pemeriksaan darah lengkap yang dapat memberikan hasil yang sangat cepat dan akurat. Pada penggunaan alat hematology analyzer apabila didapatkan nilai yang tidak normal dan terdapat informasi klinis pada pasien maka dilanjutkan pemeriksaan evaluasi apusan darah tepi (EADT) untuk mengetahui morfologi darah tepi. Morfologi eritrosit di antaranya adalah poikilositosis. Poikilositosis adalah sel darah merah yang bentuknya tidak normal. Angka kejadian poikilositosis dapat terjadi dari beberapa faktor. Poikilositosis perlu dilakukan pemeriksaan saturasi transferin dan ferritin untuk penegakan diagnosis. Pada pemeriksaan apusan darah tepi dapat dilakukan menggunakan mikroskop manual dan mikroskop digital.
Tujuan: Mengetahui korelasi persentase klas eritrosit poikilositosis dengan kadar saturasi transferin dan ferritin
Metode: Penelitian ini menggunakan desain penelitian observasional analitik dengan pendekatan cross sectional. Penelitian dilakukan di Laboratorium Patologi Klinik FK-KMK UGM Yogyakarta sejak bulan Desember 2022 sampai Februari 2023. Populasi pada penelitian ini adalah masyarakat umum yang mengikuti skrining talasemia dan subjek penelitian ini adalah peserta skrining yang sesuai dengan kriteria inklusi dan ekslusi. Korelasi antara persentase klas eritrosit poikilositosis dengan saturasi transferin dan ferritin dengan uji korelasi Spearman. Analisis statistik menggunakan SPSS versi 23. Nilai p = < 0>
Hasil: Penelitian melibatkan 122 subjek yang memenuhi kriteria inklusi dan eksklusi, dengan median usia 25 tahun (19-56 tahun).Pada penelitian ini hasil korelasi persentase klas eritrosit poikilositosis dengan saturasi transferin didapatkan nilai r = -0,202 (p < 0 xss=removed>
Kesimpulan: Penelitian ini menunjukkan semakin tinggi persentasi klas eritrosit poikilositosis, maka semakin rendah saturasi transferin dan semakin tinggi persentasi klas eritrosit poikilositosis, maka semakin rendah kadar ferritin.
Background: Hematology analyzer is an automated complete blood test tool that can provide very fast and accurate results. In the use of hematology analyzer, if abnormal values are obtained and there is clinical information on the patient, the evaluation of peripheral blood smear (EADT) examination is continued to determine the morphology of peripheral blood. Erythrocyte morphology includes poikilocytosis. Poikilocytosis is abnormally shaped red blood cells. The incidence of poikilocytosis can occur from several factors. Poikilocytosis needs transferrin saturation and ferritin examination to confirm the diagnosis. Peripheral blood smear examination can be performed using manual microscopy and digital microscopy.
Objective: To determine the correlation between the percentage class of poikilocytosis erythrocytes transferrin saturation and ferritin levels.
Methods: This study used an analytic observational research design with a cross sectional approach. The research was conducted at the Clinical Pathology Laboratory of FK-KMK UGM Yogyakarta from December 2022 to February 2023. The population in this study was the general public who participated in thalassemia screening and the subjects of this study were screening participants who met the inclusion and exclusion criteria. Correlation between the percentage of poikilocytosis erythrocyte class with transferrin saturation and ferritin with Spearman correlation test. Results: The study involved 122 subjects who met the inclusion & exclusion criteria, with a median age of 25 years (19-56 years). In this study, the correlation results of the percentage of poikilocytosis erythrocyte class with transferrin saturation had an r value of r = -0.202 (p < 0 xss=removed>
Conclusion: This study showed that the higher the percentage of poikilocytosis erythrocyte class, the lower the transferrin saturation & the higher the percentage of poikilocytosis erythrocyte class, the lower the ferritin level.
