Profil Penggunaan Obat pada Pasien Rawat Jalan Terdiagnosis Aritmia Tipe Fibrilasi Atrium di RSUP Dr. Sardjito

Atrial fibrillation is the most common type of arrhythmia, with a prevalence of 1-2%, and it is expected to continue increasing. Patient adherence to treatment and the appropriateness of the treatment according to guidelines are crucial to achieving therapeutic targets and minimizing the risk of side effects, such as bleeding. This study aims to determine the pattern of medication use in outpatient patients diagnosed with atrial fibrillation, evaluate its adherence to the guideline, identify the incidence of side effects from treatment, and patient adherence to medication.
This research is an observational study with a cross-sectional design. Data collection was conducted retrospectively. The study subjects were outpatient atrial fibrillation patients at Dr. Sardjito Hospital aged ? 18 years from July 2022 to June 2023, with minimum treatment duration of three months. Sampling was done using purposive sampling technique. Data were analyzed descriptively using Google Spreadsheet.
The results of the study on 101 patients showed that the most commonly used drugs, are warfarin prescribed to 88,12% patients and bisoprolol prescribed to 79.21% patients. The most commonly prescribed combination of drugs was a combination of beta-blockers (bisoprolol) and anticoagulant (warfarin) in 49.50% patients. Therapy evaluation results showed that 76,24% patients met the guideline criteria, with 88,11% patients met drug appropriateness and 76.24% patients met dose and frequency appropriateness based on the Atrial Fibrillation Management Guidelines by PERKI (Indonesian Heart Association) in 2014 and Lexicomp as an additional reference. Inappropriateness found included lack of antithrombotic therapy, absence of atrial fibrillation therapy, inadequate dosage, excessive dosage, and inappropriate frequency. Side effects occurred in 34,65% patients with bleeding being the most commonly found side effect, occurring in 7,92% patients, followed with coughing and dizzy in 4,95% patients. A total of 74,26% patients had good medication adherence.
