Asosiasi Antara Tumor Sumbu Terpanjang Berdasarkan CT Scan Toraks dengan Status Mutasi Epidermal Growth Factor Receptor Positif dan Negatif pada Adenokarsinoma Paru

Latar Belakang
Angka kejadian kanker paru merupakan yang tertinggi keempat di
Yogyakarta pada tahun 2017-2019. Pemeriksaan histopatologi merupakan standar
untuk mendiagnosis status mutasi adenokarsinoma EGFR yang bersifat invasif,
berisiko dan relatif lebih mahal. Fitur CT scan toraks harus digunakan
untuk menentukan status mutasi EGFR pada adenokarsinoma paru. Ukuran sumbu
terpanjang tumor adalah salah satu karakteristik yang dapat dinilai pada CT scan
toraks. Diperlukan analisis hubungan antara ukuran sumbu terpanjang tumor dan
status mutasi EGFR pada NSCLC jenis adenokarsinoma paru.
Tujuan
Penelitian ini bertujuan untuk menganalisis
asosiasi antara ukuran tumor sumbu terpanjang pada pemeriksaan CT scan
toraks dengan status mutasi EGFR pada NSCLC jenis adenokarsinoma paru.
Metode
Penelitian yang dilakukan adalah studi potong lintang dalam
mengidentifikasi secara radiologi hubungan ukuran sumbu tumor terpanjang dengan
status mutasi EGFR pada adenokarsinoma paru. Penelitian ini menggunakan data
sekunder dari pasien yang menjalani pemeriksaan transthoracic needle
aspiration (TTNA) dengan yang dipandu dengan CT scan toraks di
Instalasi Radiologi RSUD Dr. Sardjito, Yogyakarta. Sampel dikumpulkan secara berurutan
dengan memasukkan subjek secara acak yang memenuhi kriteria inklusi dan
eksklusi sampai jumlah sampel tercapai berdasarkan perhitungan jumlah sampel.

Simpulan
Background
The incidence of lung cancer was the fourth highest in Yogyakarta
in 2017-2019. Histopathological examination is the standard for diagnosing the
mutation status of EGFR adenocarcinoma which is invasive, risky, and relatively
more expensive. Thoracic CT scan features should be used to determine EGFR
mutation status in lung adenocarcinoma. The size of the longest axis of the
tumor is one of the characteristics that can be assessed on a chest CT scan.
Analysis of the relationship between tumor longest axis size and EGFR mutation
status in lung adenocarcinoma NSCLC is needed.
Purpose
This study aimed at the association between the longest axis tumor
size on thoracic CT scan examination and EGFR mutation status in adenocarcinoma-type NSCLC. Method
This study was a cross-sectional study to identify the radiological relationship between the size of the longest tumor axis and EGFR mutation status in lung adenocarcinoma. This study used secondary data from patients who underwent transthoracic needle aspiration (TTNA) examination guided by a thoracic CT scan at the Radiology Installation of Dr. Sardjito Hospital, Yogyakarta. Samples were collected sequentially by randomly entering subjects who met the inclusion and exclusion criteria until the sample size was reached based on the sample size calculation. Result
Of the 65 study subjects, 37 (56.9%) had positive EGFR mutation
status and 28 (43.1%) had negative EGFR mutation status results. The mean size
of the longest axis of the tumor was 6.39 cm with a standard deviation of 2.95
cm. In the t-test between EGFR mutation status, there was no significant
difference in the size of the longest axis of the tumor against EGFR mutation
status, but there was a significant difference in the pleural effusion
variable. Relationship analysis showed no relationship between tumor longest
axis size with EGFR mutation status with RP 1.08 (95% CI 0.356-3.293) and p
value = 0.885 based on cut-off that had been used in previous studies of
3 cm.
Conclusion
There was
no association between the size of the longest axis of tumor mass on thoracic
CT scan and EGFR mutation status in adenocarcinoma-type NSCLC.
