Hubungan Antara Status Mutasi Epidermal Growth Factor Receptors (EGFR) Dengan Karakteristik Massa, Pola Invasi, Dan Lokasi Adenokarsinoma Paru Pada Pemeriksaan CT Scan Thorax Dengan Kontras

Latar belakang: Kanker paru merupakan penyebab utama kematian akibat kanker secara global maupun di Indonesia. Status mutasi epidermal growth factor receptors (EGFR) merupakan faktor penting dalam tatalaksana adenokarsinoma paru. Masih terdapat inkonsistensi hubungan antara status mutasi EGFR dengan fitur imaging adenokarsinoma paru.
Tujuan penelitian: Penelitian ini bertujuan mengetahui hubungan antara status mutasi EGFR dengan fitur imaging adenokarsinoma paru, yaitu karakteristik massa, pola invasi, dan lokasi pada pemeriksaan CT scan thorax dengan kontras.
Metode penelitian: Penelitian ini merupakan penelitian analitik observasional dengan rancangan cross sectional. Subjek penelitian adalah pasien adenoarsinoma paru di RSUP Dr. Sardjito Yogyakarta kurun waktu Januari 2022 hingga Juni 2024 serta memenuhi kriteria inklusi dan eksklusi. Subjek yang terkumpul dikelompokkan menjadi 2, yaitu kelompok status mutasi EGFR (+) dan (-). Dilakukan analisis terhadap karakteristik massa (ukuran, kalsifikasi, spikulasi, tekstur, fissure attachment), pola invasi, dan lokasi massa pada CT scan thorax dengan kontras di kedua kelompok. Kemudian dilakukan uji chi square.
Hasil penelitian: Terkumpul 56 subjek penelitian, masing-masing 30 subjek untuk kelompok mutasi EGFR (+) dan 26 subjek untuk kelompok mutasi EGFR (-). Hasil uji statistik menunjukkan nilai p yang signifikan secara statistik pada variabel lokasi, tekstur, spikulasi, dan pola invasi dengan nilai p secara berurutan 0,035, 0,000, 0,000, dan 0,002. Nilai p tidak signifikan didapatkan pada variabel kalsifikasi, fissure attachment, dan ukuran massa.
Kesimpulan: Status mutasi EGFR berhubungan dengan lokasi, tekstur, spikulasi, dan pola invasi adenokarsinoma paru dan tidak berhubungan dengan kalsifikasi, fissure attachment, dan ukuran massa.

Introduction: Lung cancer is one of the leading causes of cancer death globally and in Indonesia. Epidermal growth factor receptor (EGFR) mutation status is a crucial factor in the management of lung adenocarcinoma. A consistent relationship between EGFR mutation status and the imaging features of lung adenocarcinoma remains elusive.
Objective: This study aimed to investigate the association between EGFR mutation status and the imaging features of lung adenocarcinoma, specifically mass characteristics, invasion patterns, and lesion location, as observed on contrast-enhanced chest CT.
Methods: This observational analytical study used a cross-sectional design. Subjects were lung adenocarcinoma patients admitted to Dr. Sardjito General Hospital in Yogyakarta between January 2022 and June 2024 and who met the inclusion and exclusion criteria. Subjects were divided into two groups: EGFR mutation status (+) and EGFR mutation status (?). Mass characteristics (size, calcification, spiculation, texture, and fissure attachment), invasion patterns, and lesion location on contrast-enhanced chest CT were analyzed in both groups. Subsequently, a chi-square test was performed.
Results: Fifty-six participants were recruited, including 30 in the EGFR mutation (+) group and 26 in the EGFR mutation (?) group. Statistical analysis showed significant associations for tumor location, texture, spiculation, and invasion patterns, with p-values of 0.035, <0>
Conclusion: EGFR mutation status is associated with tumor location, texture, spiculation, and invasion patterns of lung adenocarcinoma but not with calcification, fissure attachment, or tumor size on contrast-enhanced chest CT.
