THE EFFICIENCY AND SAFETY OF HONEY AS A TOPICAL TREATMENT FOR ACUTE BURN WOUND COMPARED TO SILVER SULFADIAZINE : A SYSTEMATIC REVIEW

Latar Belakang: Luka bakar adalah luka pada kulit atau jaringan organik lain yang disebabkan oleh panas. Jumlah kematiannya sekitar 180 ribu per tahun dan paling banyak terjadi di daerah Afrika dan Asia Tenggara. Luka ini biasa diobati dengan Silver Sulfadiazine karena SSD adalah standar emas untuk merawat luka bakar. SSD mendapatkan reputasi demikian karena SSD terbukti efisien dalam menyembuhkan luka bakar dan bakterisida. Namun, banyak alternatif SSD yang terbukti lebih baik daripada SSD dalam menyembuhkan luka bakar, seperti madu.
Tujuan: Membandingkan efisiensi, keamanan, dan aplikasi madu terhadap SSD.
Metode: Studi ini merupakan systematic review menggunakan situs website Pubmed, Elsevier, Google Scholar, Cochrane, dan Scopus, ditemukan 457 studi. Setelah dilakukan seleksi duplikasi studi dan skrining berdasarkan kriteria inklusi dan eksklusi, ditemukan lima studi yang akan di analisis. Hanya empat studi yang memiliki data keluaran berupa epitelialisasi dan dua studi yang memiliki data keluaran nyeri, kedua keluaran tersebut kemudian dianalisa secara statistik menggunakan SPSS 28.0. Sementara tiga studi terkait aplikasi akan didiskusikan secara deskriptif. Kelima studi kemudian di analisa menggunakan CEBM RCT Critical Appraisal Sheet.
Hasil: Hasil studi ini menunjukkan bahwa madu lebih maju dibandingkan SSD dalam hal efisiensi, keamanan, dan aplikasi. Madu lebih cepat dalam hal epitelialisasi secara statistik dibandingkan dengan SSD, namun tidak secara klinis pada terapi luka bakar. The overall estimate effects menunjukkan angka 1.46 (95% IK [1.15;1.86], p=<0 class="s1" xss=removed>2=80%). Madu juga lebih aman dalam hal nyeri dibandingkan dengan SSD, namun tidak secara klinis. The overall estimate effects menunjukkan angka1.14 (95% IK [1.01;1.30], p=0.04, I2=26%). Terakhir, madu lebih murah dibandingkan dengan SSD dalam terapi luka bakar.
Kesimpulan: Madu secara statistik lebih cepat dalam hal epitelisasi, lebih aman dalam hal nyeri, dan lebih murah dibandingkan dengan SSD dalam terapi luka bakar.

Background: Burn wound is an injury to the skin or other organic tissue caused by heat. Its death toll is around 180,000 annually and frequently occur in Africa and South East Asia. This injury is usually treated with Silver Sulfadiazine since SSD is the gold standard when it comes to treating burn wounds. It earns its place because it’s proven to be efficient in healing burn wounds and bactericide. However, there are lots of alternatives to SSD that are proven to be better in treating burn wounds, such as honey.
Objective: Comparing the efficiency, safety, and applicability of honey towards SSD.
Method: This study is a systematic review using Pubmed, Elsevier, Google Scholar, Cochrane, and Scopus, was found 457 studies. Following the removal of duplicates and screening based on eligibility criteria, resulting in five studies to be analyzed. Only four studies provided with rate of epithelialization as the outcome and two studies with pain relief as the outcome, both were analyzed statistically using SPSS 28.0. While three studies regarding applicability as the outcome were then compared descriptively. All five studies were assessed with CEBM RCT Critical Appraisal Sheet.
Result: Our study showed that honey surpassed SSD in terms of efficiency, safety, and applicability. Honey is statistically significant faster in epithelialization compared to SSD, but not clinically significant in treating burn wounds. The overall estimate effects is about 1.46 (95% CI [1.15;1.86], p-value=<0 class="s1" xss=removed>2=80%). Honey is also statistically significant safer in pain relief compared to SSD, but not clinically significant. The overall estimate effects is about 1.14 (95% CI [1.01;1.30], p-value=0.04, I2=26%). Lastly, Honey has a significantly lower cost compared to SSD in treating burn wounds.
Conclusion: Honey is statistically significant faster in epithelialization, safer in pain relief, and cheaper compared to SSD in treating burn wounds.
