Efektivitas Kombinasi Pemberian Topikal Madu Kaliandra dan Terapi Musik Klasik Terhadap Penyembuhan Ulkus Diabetik dan Tingkat Kenyamanan di Sorong, Papua Barat

Latar Belakang: Ulkus diabetikum pada pasien diabetes melitus yang tidak dirawat dengan baik akan menyebabkan dilakukannya tindakan amputasi dan penurunan tingkat kenyamanan. Penelitian sebelumnya telah melihat pengaruh madu kaliandra dan pengaruh terapi musik klasik dalam meningkatkan penyembuhan ulkus diabetikum dan tingkat kenyamanan. Namun perawatan yang mengkombinasi kedua terapi tersebut belum dilakukan.
Tujuan: Mengetahui efektivitas kombinasi pemberian topikal madu kaliandra dan terapi musik klasik terhadap penyembuhan ulkus diabetikum dan tingkat kenyamanan pasien.
Metode: Penelitian ini merupakan penelitian quasi experimental design (pretest and posttest nonequivalent control group). Sebanyak 52 pasien DM yang memiliki ulkus diambil dengan consecutive sampling dan dibagi menjadi dua kelompok. Penelitian ini menggunakan Bates-Jensen Wound Assessment dan General Comfort Questionnaire. Data dianalisis menggunakan uji Repeated Anova/GLM-RM untuk membandingkan skor penyembuhan ulkus dan Mann Withney Test untuk skor kenyamanan serta uji Regresi Linear Berganda sebagai analisis Multivariat.
Hasil: Hasil penelitian menunjukkan bahwa ada perbedaan yang signifikan terhadap skor penyembuhan ulkus diabetikum (p value = 0,035) dan skor tingkat kenyamanan (p value = 0,000) antara kelompok intervensi dan kelompok kontrol. Hasil uji multivariat menunjukkan tidak ada hubungan variabel luar dengan penyembuhan ulkus diabetikum (p value > 0,05). Effect size untuk penyembuhan ulkus diabetikum 0,139 dan 0,178 untuk tingkat kenyamanan.
Kesimpulan: Kombinasi madu kaliandra dan terapi musik klasik efektif terhadap penyembuhan ulkus diabetikum dan tingkat kenyamanan pasien dibandingkan dengan pemberian madu kaliandra tanpa terapi musik klasik, serta tidak ada hubungan variabel luar dengan penyembuhan ulkus diabetikum.
Background: Diabetic ulcers in patients with diabetes mellitus who are not treated properly will result in amputation and decreased comfort levels. Previous research has looked at the influence of Calliandra honey and the influence of classical music therapy in improving diabetic ulcer healing and comfort levels. However, treatments that combine both therapies have not been carried out.
Purpose: To determine the effectiveness of a topical combination of Calliandra honey and classical music therapy in healing diabetic ulcers and the patient's comfort level.
Methods: This study used a quasi-experimental design (pretest and posttest nonequivalent control group). The participants were 52 diabetes patients who had diabetic ulcers taken using consecutive sampling and divided into two groups. This study used the Bates-Jensen Wound Assessment (BWAT) and General Comfort Questionnaire (GCQ). Data were analyzed using the Repeated Anova/GLM-RM test to compare ulcer healing scores and the Mann Withney Test to compare comfort scores between the intervention group and the control group and the Multiple Linear Regression test for Multivariate analysis.
Results: The results showed that there was a significant difference in diabetic ulcer healing scores (p value = 0.035) and comfort level scores (p value = 0.000) between the intervention group and the control group. Multivariate test results showed that there was no relationship between external variables and healing of diabetic ulcers (p value > 0.05). Effect size for healing diabetic ulcers 0.139 and 0.178 for comfort.
Conclusion: The combination of calliandra honey and classical music therapy is effective in healing diabetic ulcers and the patient's comfort level is compared with giving calliandra honey without classical music therapy, and there is no relationship between external variables and healing of diabetic ulcers.
