Analisis Critical Success Factors (CSF) Pada Penerapan Clinical pathways Di RSUD Tidar Magelang

Latar Belakang : Rumah sakit merupakan salah satu lembaga penyeda pelayanan kesehatan. Dalam memberikan pelayanan rumah sakit menggunakan CP sebagai tools untuk mewujudkan good clinical governance di rumah sakit. Selain itu, CP menjadi salah satu komponen penilaian berdasarkan Standar Akreditasi Rumah Sakit versi KARS 2022. Berdasarkan hasil studi pendahuluan pada penerapan CP terdapat beberapa kendala yang ditemukan pada masa uji coba CP yang berlangsung pada akhir Desember 2022 – Juni 2023. Oleh karena itu, perlu dilakukan identifikasi faktor kritis penentu keberhasilan penerapan clinical pathway di RSUD Tidar agar penerapan clinical pathway berjalan sesuai dengan tujuan yang diharapkan.

Tujuan : Mengidentifikasi Critical Success Factors (CSF) pada penerapan CP.

Metode : Penelitian ini merupakan penelitian mix methode yaitu metode kualitatif untuk  mendapatkan informasi mendalam terkait penerapan CP berdasarkan model implementasi kebijakan Van Metter dan Van Horn dilanjutkan dengan menentukan CSF menggunakan metode kuantitatif dengan perhitungan Analytical Hierarchy Process (AHP). Informan pada penelitian ini adalah tim CP, case manager, admin bangsal, perawat, dan casemix manager. Responden pada penelitian berjumlah 9 orang. Pemilihan informan dan responden menggunakan teknik purposive sampling. Objek penelitian adalah laporan trwiulan kepatuhan CP dan dokumen CP. Lokasi penelitian di RSUD Tidar Magelang dan berlangsung pada bulan April- Agustus 2023. Pengumpulan data menggunakan teknik wawancara dan penyebaran kuisioner. 

Hasil : Faktor kritis keberhasilan penerapan clinical pathway di RSUD Tidar Magelang yang dapat teridentifikasi adalah menentukan capaian dan sasaran penerapan clinical pathway dengan bobot nilai 35,17%, menyelenggarakan sesi diskusi dari antar PPA untuk menyamakan pemahaman dalam pengisian clinical pathway dengan bobot nilai 53,78%, membuat electronic clinical pathway dengan bobot nilai 54,48%, meningkatkan komitmen PPA dan seluruh bagian terkait pada pelaksanaan clinical pathway dengan bobot nilai 46,48%.

Kesimpulan : Faktor kritis keberhasilan penerapan CP dapat dijadikan sebagai strategi untuk menerapkan clinical pathway yang disesuaikan dengan kondisi dan kebutuhan rumah sakit.

Background: The hospital is one of the health service providers. In providing hospital services, using CP as a tool to realize good clinical governance in hospitals. In addition, CP is one of the assessment components based on the KARS 2022 version of the Hospital Accreditation Standard. Based on the results of preliminary studies on the application of CP, there are several obstacles found during the CP trial period which will take place at the end of December 2022 – June 2023. Therefore, it is necessary to identify critical factors determining the success  of  clinical pathway implementation at Tidar Hospital so that the implementation of clinical pathway runs in accordance with the expected goals.

 Objective: Identify Critical Success Factors (CSF) in the application of CP.

Method : This research is a mix method research, which is a qualitative method to obtain in-depth information related to the application of CP based on the Van Metter and Van Horn policy implementation model followed by determining CSF using quantitative methods with Analytical Hierarchy Process (AHP) calculations. The informants in this study were the CP team, case manager, ward admin, nurse, and casemix manager. Respondents in the study amounted to 9 people. Selection of informants and respondents using purposive sampling techniques. The object of study is the CP compliance review report and the CP document. The research location is at Tidar Magelang Hospital and will take place in April-August 2023. Data collection using interview techniques and questionnaire dissemination. 

Results: The critical factors  for the success of clinical pathway  implementation at Tidar Magelang Hospital that can be identified are determining the achievements and targets of  clinical pathway implementation with a value weight of 35.17%, holding discussion sessions from between PPAs to equalize understanding in filling  the clinical pathway with a value weight of 53.78%, making an electronic clinical pathway with a value weight of 54.48%, increasing the commitment of PPA and all related sections in the implementation of the clinical pathway with a value weight of 46.48%.

Conclusion : The critical success factor of implementing CP can be used as a strategy to implement a clinical pathway tailored to hospital conditions and needs.
